2023-1220-Request for Certificate of Compliance-176 Turkey Hill Road.pdf     12.20.2023  Sarah LaValley  Conservation, Preservation and Land Use Planner  Northampton Office of Planning and Sustainability  210 Main Street  Northampton, MA 01060    Re: Request for Certificate of Compliance  176 Turkey Hill Road Northampton, MA  Parcel 34‐016  Owners: Linda Doucette‐Ashman and Harvey Ashman  Site Assessor Map ID: Parcel 34‐016  Book and Page: BK 13666‐ Pg 341  DEP File #: 246‐0751  Orders Recording #: Bk 14286‐ Pg 129    Dear Sarah,    We are pleased to submit this letter requesting a Certificate of Compliance relating to the  Notice of Intent filed March 3, 2021, the Order of Conditions dated April 14, 2021 and the  clarification of condition #34 at the Conservation Commission hearing on May 13, 2021  (collectively “Order of Conditions”) on behalf of owners Linda Doucette‐Ashman and Harvey  Ashman at 176 Turkey Hill Road.  This project involved the demolition of an existing single‐ family home and building of a new single‐family home.    Berkshire Design conducted multiple site visits throughout construction and observed the  work in progress. The site was stabilized throughout construction: silt fences, and barriers  were maintained, and the resource areas were protected. Final finish work, seeding and  planting took place during the Summer of 2023. As of Fall of 2023, the limit of work associated  with the project is fully stabilized.  At this time, maintenance of silt fences has ceased and  removal of fencing is pending authorization provided by the Conservation commission.  All  areas upslope of the fencing is 100% stable with uniform vegetative cover.    The following documents accompany this request:    1. WPA Form 8A – Request for Certificate of Compliance    2. Letter from Don Teagno, President, Teagno Construction Incorporated dated  December 15, 2023 as general contractor on the project (a) confirming the Order of  Conditions has been satisfactorily completed, and (b) reporting that condition #36 has  been satisfactorily completed as further described in the letter.    3. Record Conditions Plan of Land dated October 2, 2023 signed and stamped by  registered professional land surveyor, Timothy D. Armstrong, and registered  Carolyn Misch, Director  City of Northampton Office of Planning & Sustainability  Berkshire Design Group                  176 Turkey Hill Road ‐Request for Certificate of Compliance  │  Page 2                          professional engineer, Greg P. Henson, showing the as‐built drainage structures and  other stormwater structures and elevations within 100 feet of wetland boundaries as  required by condition #32.     4. Affidavit from Greg P. Henson, Civil Engineer about the installation of stormwater  features.     6. Site Construction Erosion Control Plan LC122 dated May 10, 2021 as presented to the  Conservation Commission and approved at a hearing on May 13, 2021.  This  document clarified:     a. the limit of work line would be at the tree line (rather than at the 50 foot  buffer zone line) in the area of wetland flags B38 to B45;    b. the lawn would be restored up to the tree line; and     c. the triangular shaped area shown on this plan between flags B43 and B45 will  be a “no mow zone” and not maintained as lawn.    These changes supersede the requirements of condition #34 in the Order of  Conditions dated April 14, 2021.      Based on the above, I hereby certify that the project followed substantial compliance with the  plans, and the project is completed.  We respectfully request the Northampton Conservation  Commission issue a Certificate of Compliance for the project at 176 Turkey Hill Road.     If you have questions or need anything else, feel free to call or email.      Sincerely,         Rachel     Rachel Loeffler, RLA, ASLA, Principal  Berkshire Design Group, 413‐582‐7000 x 119  wpaform8a.doc •• rev. 5/29/14 Page 1 of 2 Massachusetts Department of Environmental Protection Bureau of Resource Protection - Wetlands WPA Form 8A – Request for Certificate of Compliance Massachusetts Wetlands Protection Act M.G.L. c. 131, §40 DEP File Number: 246-0751 Provided by DEP A.
