595 Haydenville Road_Request for Certificate of Compliance.pdfAn Equal Opportunity Employer M/F/V/H January 18, 2024 GZA File No. 15.0167167.00 Northampton Conservation Commission 210 Main Street, Room 11 Northampton, MA 01060 Re: Request for Certificate of Compliance DEP # 246-0754 Bridge Replacement – Driveway Access 595 Haydenville Road Leeds, Massachusetts Dear Conservation Commission Members: On behalf of Seth and Angie Gregory, GZA GeoEnvironmental, Inc. (“GZA”) respectfully requests a Certificate of Compliance for MassDEP file Number 246-0754. The project involved the replacement of a residential access bridge at 595 Haydenville Road in Northampton, Massachusetts (“Site”). GZA conducted a site visit on November 9, 2023, to observe the completed project conditions as com- pared to those approved by the Order of Conditions (OOC) issued by the Northampton Conservation Commission (Commission) on October 27, 2021. During the site visit, GZA collected GPS points of the bridge and driveway footprint, and the estimated boundary of an adjacent wetland resource area using a sub-meter accuracy collection device. The locations of GPS-surveyed features are depicted on the As-Built Plan in Appendix A. The bridge appears to have been built within the proposed limits shown on the approved plan included in the OOC. Construction was completed on October 19, 2023, and all disturbed areas were seeded and covered with straw. GZA conducted the aforementioned follow-up Site visit on November 9, 2023 and observed vegetation growth in the seeded areas. These areas had enough vegetative cover growing beneath the straw to consider the Site sufficiently stabi- lized (Photo 1). During the project construction phase, GZA was on-site daily to conduct turtle sweeps ahead of construction activity, per the re- quirements in the NHESP-approved Wood Turtle Protection Plan. All site visits were documented with photographs. No wood turtles were observed within the on-site work area, or adjacent to the work area, dur- ing GZA’s inspections. January 18, 2024 GZA File No. 15.0167167.00 595 Haydenville Road – Request for Certificate of Compliance Page | 2 Proactive by DesignBased on GZA’s review of the current Site conditions, it appears as though the completed work is substantially compliant with the issued Order of Conditions. Therefore, on behalf of Seth and Angie Gregory we respectfully request a Certificate of Compliance for the Project.
