Approval of the Stormwater Management Permit Application subject to any conditions, modifications or restrictions requi><•ed by the Department of Public Works (see below) Disapproval of the Stormwater Management Permit Application based upon a determination that the proposed plan, as submitted, does not meet the purposes set forth in the Northampton Stormwater Management. Standard Permit Conditions: All clearing, grading, drainage, construction, and developmetzt shall be conducted in strict accordance with the following plans and information submitted with the application and as amended by any conditions attached to this permit. a. Village at Hospital Hill-Phase One, Site Plans, Sheets L-103 Grading and Utilities Plan and L-AD 1 Road Profile, dated 5/4/2005 by Dietz & Co. Architects, Inc. and The Berkshire Design Group. b. Proposed Stormwater Management System Operation & Maintenance Plan for the Village at Hospital Hill, Phase I Site, dated 7/13/2004 by TCB Hospital Hill, LLC. d. Drainage Report for Village at Hospital Hill Phase I Site Plan, dated June 2004 by The Berkshire Design Group. e. Drainage Calculations for Basin 4 Revisions, dated 3/10/2005 by The Berkshire Design Group f. Stonnwater Pollution Prevention Plan for Village at Hospital Hill-Phase I, dated March 1, 2005, signed by The Community Builders, Inc., Aquadro & Cerutti, Inc., and Dietz Construction. 2. The Northampton Department of Public Works (DPW) or its authorized representative shall be provided access to the property to conduct inspections before, during, and after construction activities as necessary. The applicant shall arrange with the DPW to schedule the following inspections: Initial Inspection, Erosion Control Inspection, Bury Inspection, and Final Inspection. The DPW may also conduct random inspections to ensure effective control of erosion and sedimentation during all phases of construction. 3. Annual Reports that are stamped by a Registered Professional Engineer documenting the actions completed as required by the Operation, Maintenance and Inspection Agreement shall be submitted to the Northampton Department of Public Works by November 1St of each year. - - - _ - _ -- - - - 4. If construction of the proposed Stonnwater system has not started within five years of the issue date of this permit, then the permit will expire and a new permit will be required. Construction is defined as clearing, grading, excavation, filling, and installation of drainage structures on the lots specified in this permit. Specific Permit Conditions: 1. Prior to issuance of a certificate of occupancy, the applicant must record an executed Stonnwater Operation, Maintenance, and Inspection Agreement that is binding on all subsequent owners of land served by the private stonnwater management facility by recording it in the land records of the Registry of Deeds. This agreement shall be signed by the applicant and approved by the Northampton DPW. Section 22-85 of the Northampton Stonnwater Management Ordinance provides additional guidance for the requirements of this agreement. 2. The applicant shall submit to the Northampton DPW within 30 days of the issuance of this pernzit, final plans and stormwater calculations documenting the existing and proposed site development and the expected flows from these areas. The stonnwater calculations shall be stamped by a Registered Professional Engineer and use identical design stones as the Beals and Thomas, Inc. stonnwater calculations approved for the Phase I Definitive Subdivision Plan for this site. )/:\Storniwater Permits\Stonnwater Permits\Pennits_new applications\Village at Hosptial Hill Phase I.doc Page 2 of 3 3. The applicant shall submit to the Northampton DPW within 30 days of the issuance of this permit, documentation that the capacity of the proposed 24 inch drain line to Detention Basin 4 is adequate to convey all the flows from the planned site build-outs of all the areas in drainage area (PDA) 2C that were not proposed to be connected to the previously approved 24 inch drain line as part of the Phase I Definitive Subdivision Plan. The applicant shall also submit an explanation why these two 24 inch drain lines cannot be combined. Signature of City Engineer Date cc.
