B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional

Dear Commissioners:
Enclosed please find WPA Form 8A – Request for Certificate of Compliance related to the above-referenced
project. Also enclosed are the As-Built Plans of the project and photos of the completed site work.
Per the plans, six (6) Clethra alnifolia, sweet pepper bush, and five (5) Matteuccia struthiopteris, ostrich fern,
were installed. Japanese knotweed infestation was recently cut, and stumps removed in areas where it was
feasible to not disturb the bank. No herbicide was used in the invasive plant control activities.
The original front porch and footings were removed, and the new side porch and deck and associated footings
were built according to plan and elevation.
I certify that a competent person under my supervision has inspected the work and found that it has been
constructed in substantial conformance with the Order of Conditions issued on 1/27/2022.
