Dear Carl,
We would like to engage Manna Soup Kitchen to manage and coordinate our public lockers (E.
John Gare Parking Garage, 85 Hampton Avenue) for 22 weeks from the date of the installation of
the lockers for a $1,000 flat rate.
The work includes
Assign lockers to users who need them for storage and maintain a waiting list (if needed)
Establising clear locker use guidelines and ensure users understand those guidelines
Establish a process for lost codes and access when that is necessary
Regulary inspect and clean lockers, not less than once a month
Alert the city (Alan Wolf, Mayor Chief of Staff) if maintenance is required

We accept the above terms and agree to indemnify and hold the city harmless from any work
performed by Manna, its employees, volunteers and assigns (but not third parties or locker users).
We will provide evidence that Manna carries the required insurance:
Workmen's Compensation and other benefits (MGL C 152 and MCL C 149, Section 34A);
Personal Injury and Accidental Death - General Liability (Each person/aggregate- $300,000/$1,000,000);
Property Damage - General Liability (Each Occurrence/aggregate- $500,000/$1,000,000)
