changed to “(k) reconstruction of road drainage structures”
2. Notice of Intent Application (WPA Form 3, page 3), Part B. Buffer Zone and Resource Area
Impacts has been modified to include estimated linear feet and area of impacts to “land
under water” and “bank”, respectively.
3. Notice of Intent Application (WPA Form 3, page 7), Part C. Other Applicable Standards and
Requirements: Item 7 has been modified -- 7a is checked (Stormwater Report required);
and Item 7a 1 – is checked (site constitutes redevelopment).
4. Notice of Intent Application (WPA Form 3, page 8), Part D. Additional Information: Item 9 is
checked (Attach Stormwater Report)
5. Addendum, Part 2.6 has been modified to report 2,160+/- SF of existing land under water
temporary impacts and permanent impact of additional 460 SF of land under water
