Massachusetts Wetlands Protection Act, G.L. c, 131 §40
1. 1, the undersigned, hereby request that the NORTHAMPTON
Conservation Commission make a determination as to whether the area, described below, or work to
be performed on said area, also described below, is subject to the jurisdiction of the Wetlands
Protection Act, G.L C. 131, §40.
2 The area is described as follows. (Use maps or plans, if necessary, to provide a description and the
location of the area subject to this request) A residence located at 220 Main Street
Leeds as shown on Map #10, Lot #84 of the Northampton Assessors Map and
as shown on attached Plot Plan.
3. The work in said area is described below. (Use additional paper, if necessary, to describe the
proposed work.)
The construction of a 2' 6" cantilevered Bay Window ex - tensl6n to existing
dining room, as shown on attached plans. The construction does not involve

extension and the existing dining room will be raised approximately 8" to
match the height of the existing living space.

Dear Sir:
On May 19, 1989, representatives of this firm ran trigonometric
levels from the bench mark on the northeast corner of the Mulberry
Street Bridge to the above address.
We determined that the sill elevation is 334.50 feet and the
flood hazard elevation is at 334.0 in this vicinity. These
elevations are based on N.V.G.D. datum.
Please note that R M 3 on the FEMA flood hazard map states that
this TBM is located on the southwest abutment. We found no
mark on the southwest abutment.

NORTHAMPTON CONSERVATION COMMISSION
The Northampton Conservation Commission will hold a Public
Meeting, on Monday evening May 22, 1989 at 10:00 PM in Council
Chambers, Wallace J. Puchalski Municipal Office Building, 212 Main
Street, Northampton, as required by MGL Chapter 131, Section 40,
Wetlands Protection Act on the Request for Determination filed by:
Michael and Margaret MacDonald for work done expanding an existing
dining room in a floodplain and in the buffer of a resource area.
All work is on a parcel of land located at 220 Main Street, Leeds,
more particularly identified as Parcel 84 on Northampton Assessor's
Map #10.
