The purpose of this form is to make sure that all project applications applying for Community
Preservation Act funding are eligible for funding. Please refer to Appendix-E in the
Community Preservation Plan when filling out this form. This form must be approved in
order for an application to be accepted.

Project Summary: Please provide a brief description of the project.
First Churches has a total of 13 stained glass windows that are deemed high priority for
restoration and repair. A courtesy estimate from Stained Glass Resources, Inc. identified
the following needs, in order of most critical importance:
1) The River of Life Tiffany Window needs total restoration, re-leading and cleaning,
replacement of wooden frame and protective overglaze. Est. $95,000
2)The Rose Window, a large window facing Main Street, also needs restoration and
glazing. Est. $116,000 including scaffolding
3)The Road to Emmaus Window re-leading and restoration Est. $37,500
4)Other general window re-leading and glazing Est. $60,000
5)Architect fees, travel and consulting (as per State Historical Commission Estimate)
$15,000 Total scope of work = $323,500
