WORKS HAS BEEN TAKEN ON THIS APPLICATION FOR AN AMENDMENT OF AN
ACTIVE PERMIT BASED ON INFORMATION PRESENTED:

conditions, modifications or restrictions required by the Department of
Public Works (see below)
__ Disapproval of the Stormwater Management Permit Amendment based upon
a determination that the proposed plan, as submitted, does not meet the
purposes set forth in the Northampton Stormwater Management.
Standard Permit Conditions:
I. All clearing, grading, drainage, construction, and development of the proposed Coca Cola
Wastewater Treatment System Modifications shall be conducted in strict accordance with
the previously approved plans and information as referenced in the Stormwater
Management Permit issued by the Northampton Department of Public Works (DPW) on
January 14, 20 I 0 and as amended by the following plans and information and as
amended by any conditions attached to this permit amendment.

Specific Permit Conditions:
1. In accordance with Specific Permit Condition 1 of the Stormwater Management Permit
issued on January 14, 2010 a Stormwater Operation, Maintenance, and Inspection
Agreement was recorded on 10/25/2011 (See Book10697/Page 87, Hampshire Registry
of Deeds). Prior to the start of construction and issuance of a building permit, the
applicant must amend the previously recorded Stormwater Operation, Maintenance, and
Inspection Agreement to include all inspection and maintenance tasks for the proposed
stormwater management system. A draft of the proposed revisions must be submitted to
the DPW for approval prior to recording of the changes.
2. Prior to construction of the infiltration systems, a report shall be submitted to the DPW
by a Certified Soil Evaluator or Certified Professional Soil Scientist documenting the
soils and elevation of seasonal high ground water in the location of the proposed
infiltration system and confirming the proposed rate of infiltration and the appropriate
separation to ground water for the proposed infiltration system.
3. During construction and prior to backfill of the proposed subsurface infiltration system,
the applicant or the contractor shall notify the DPW for inspection of the system.
4. Additional erosion and sediment control measures and details are necessary for the
project including, but not limited to: stabilized construction access (tracking pads),
protection of the infiltration system from soil compaction and sedimentation during
construction, and other measures as necessary. A revised Erosion Control Plan that
includes all details and specifications shall be submitted to the DPW for approval prior to
the start of construction.
