Standard Permit Conditions: 1. All clearing, grading, drainage, construction, and development shall be conducted in strict accordance with the following plans and information submitted with the application and as amended by any conditions attached to this permit: a. Smith College Health Services Building, Site Plans, Sheets L-1 A, L-1 B, L-6 and L-7 dated 9/26/2013, Sheets L-3, L-4 and L-5 dated 10/10/2013, stamped by Mark B. Darnold, P.E. and Richard M. Klein, R.L.A., The Berkshire Design Group, Inc. b. Stormwater Drainage Report for Smith Colleg~ New Wellness Center, Northampton, MA (including full drainage calculations), Revised 10/10/2013, Prepared by The Berkshire Design Group, Inc. The Northampton Department of Public Works (DPW) or its authorized representative shall be provided access to the property to conduct inspections before, dUTing, and after construction activities as necessary. The applicant shall arrange with the DPW to schedule the following inspections: Initial Inspection, Erosion Control Inspection, Bury Inspection, and Final Inspection. The DPW may also conduct random inspections to ensure effective control of erosion and sedimentation during all, phases of construction. 3. At the completion of the work, including construction of stormwater management facilities and landscaping, the applicant shall submit to the DPW ~s-built plans stamped by a professional engineer of record in the Commonwealth of Massachusetts and a statement from a professional engineer of record in the Commonw~alth of Massachusetts certifying that the materials and construction. of the stoqnwat(;)r management system was constructed in accordance with the approved plans and will function a~ designed. All changes to the approved stormwater facilities shall be documented by a professional engineer of record in the Commonwealth of Massachusetts and approved by 1he DPW prior to submission of as- . built plans and the certification statement described above. Annual Reports that are stamped by a Registered Professional Engineer documenting the actions completed as required by the Operation, Maintenance and Inspection Agreement shall be submitted to the Northampton Department of Public Works by October 1'1 of each year (see Specific Condition I below). · · " 5. If construction of the proposed .storm water system has not started within five years of the issue date of this permit, then the permit will expire and. a new permit will be required. Construction is defined as clearlrig, grading, excavation, fiiling, and installation of drainage structures on the lots specified in this permit. \. l '" ' .,, ,,,,. Specific Permit Conditions: I. Prior to the start of construction or issuance of a Buildiilg Permit, the applicant shall record an executed Stormwater Operation, Maintenance, and I~spection Agreement that is binding on all subsequent owners ofland served by the private stormwater management facility by recording it in the land records of the Registry of Deeds. The maintenance schedule and responsibility for maintenance of all components of the proposed storm water management system must be adequately specified in Attachment A to this '.Agreement. The Proposed Stormwater Management Plan System Operation and Maintenance Post-Construction submitted in the Stormwater Drainage Report must be revised as necessary and approved by the Northampton DPW. This agreement shall be approv¢d by the Northampton DPW and signed by the owner and the Mayor ofNorthampton. Chapter 281 of the Northampton Stormwater Management Ordinance provides additional guidance for the requirements of this agreement. 2. This project will be required to submit one or more Notice ofintent (NOI) to the U.S. EPA for coverage under the NPDES Sto1mwater Construction General Permit. A copy of the 'final NOI and Stormwater Pollution Prevention Plan (SWPPP) shall be submitted to the M:\Stormwater Management Permits\Belmont Ave_Smith Health Services\Belrnont Ave_ Smith Health Services_Permit.docx, Page 2 of3 Northampton DPW for approval prior to the start of construction and prior to issuance of a building permit. 3. The proposed stonnwater management system is designed to mitigate peak stormwater flows. If, after construction of the proposed system, the system is found to increase sto1mwater flows to adjacent properties or to the municipal storm drain system, the owner shall redesign and reconshuct the drainage system to prevent any increased stormwater flows to adjacent properties or the municipal storm drain system. 4. Sheet L-4 shows a proposed connection of the new drainage system to an existing 12 inch RCP drain line to the municipal drainage system in Belmont Avenue. During construction of the proposed drainage connection, information must be submitted to the DPW regarding the condition, slope, existing connections, and expected function and capacity of the existing drain line to Belmont A venue. 5. Prior to the start of construction of the proposed subsurface dry wells and subsurface detention basins, the applicant shall: a. Notify the DPW of the expected start date of construction of these structures for inspection by the DPW 6. Prior to the start of construction, construction plans, stamped by an engineer of record in the commonwealth of Massachusetts, shall be submitted to the DPW for approval. The conshuction plans shall include the following revisions: a. The proposed subsurface detention system must have adequate access for inspection and maintenance. The construction plans must clearly specify the location of all risers and clean outs for maintenance. b. The construction plans and documents must include a specification of clean, washed stone for the proposed subsurface detention system. The construction plans, calculations, and specifications for the proposed drainage system must be consistent with each other and consistent with plans approved by the Northampton Planning Board. cc.
