Darnold, P.E., The Berkshire Design Group, with the note: Record As-Built Drawing The Berkshire Design Group, Inc. visually field verified that the location and installation of the drainage structures have been installed as depicted on this Record Drawing. The Berkshire Design Group did not conduct any field measurements of the completed site, and all rim and invert elevations shown on this Record Drawing were provided to Berkshire Design Group by the Site Contractor, Ernest Guigli & Sons, Inc. The Stormwater Management Permit issued for this project on 10/24/2013 included the following condition: 3. At the completion of the work, including construction of stormwater management facilities and landscaping, the applicant shall submit to the DPW as-built plans stamped by a professional engineer of record in the Commonwealth of Massachusetts and a statement from a professional engineer of record in the Commonwealth of Massachusetts certifj;ing that the materials and construction of the stormwater management system was constructed in accordance with the approved plans and will function as designed. All changes to the approved stormwater facilities shall be documented by a professional engineer of record in the Commonwealth of Massachusetts and approved by the DPW prior to submission of as- built plans and the certification statement described above. The DPW requires that revised as-built plans as described in the condition above be submitted to the DPW for review. A professional engineer must stamp the plans and make a certification statement as described in the condition above. All survey, submittals, and other information necessary for the professional engineer to stamp and certify the as-built plans should be Pg.1/2, M:IStormwater Permits\Belmont Ave_ Smith Health Services\Smith Health Services As-Built Letter_draft 20140911.docx completed. It is necessary for Berkshire to take field measurements and surveys to document the stormwater systems as they were constructed. If any changes exist between the approved plan and the constructed systems these need to be documented by Berkshire.
