The purpose of this form is to make sure that all project applications applying for Community
Preservation Act funding are eligible for funding. Please refer to Appendix-E in the
Community Preservation Plan when filling out this form. This form must be approved in
order for an application to be accepted.

Project Summary: Please provide a brief description of the project.
Construction funding for the Pulaski Park renovation and “overlook”
expansion. Funding includes construction administration professional services
from Stephen Stimson Associates. Project will be built in 2 construction
phases. Newly added to the construction funding is an allowance to move all
overhead utilities (telephone, electric, fiber optic, cable) to be underground.
