The purpose of this form is to make sure that all project applications applying for Community
Preservation Act funding are eligible for funding. Please refer to Appendix-E in the
Community Preservation Plan when filling out this form. This form must be approved in
order for an application to be accepted.

Project Summary: Please provide a brief description of the project.
The Conservation Commission plans to apply for CPA funds to purchase a 50-acre parcel
in the Saw Mill Hills. Directly adjacent to 2013's acquisition, this parcel includes a maple
sugar house, open field, streams and wetlands, and a network of already developed
hiking trails. CPA funds are hoped to be used as a match for a Local Acquisitions for
Natural Diversity Grant that will be announced this fall.
