Questions and Answers Housing Supportive Services - Northampton Housing Partnership 1)What measures of success will be brought to bear in 3 years when continued funding will be needed? Firstly, the universe of support services to which referrals will be made can be  identified , described and documented. Collaborations will be pursued. The success of this program will be largely dependent on the agency/person hired  for this work and their knowledge of the system, the resources available and the linkages that can be made to help stabilize the housing for at- risk households The degree of collaboration with agencies in the support network can be  documented. Western Mass Regional Network to End Homelessness is a networking resource where outreach can occur and working relationships made. This project will address individual household situations and provide general  information sessions for targeted households, macro and micro interventions. Data will be tracked for the numbers of households stabilized, as in evictions  prevented/housing stabilized as a result of the assistance provided. Information will be kept on the trainings and workshops conducted, the numbers  who attended and follow up done with households after the sessions. Success will be measured by looking at the total numbers of households at risk of  homelessness served, as identified by service provider and court referrals, who were able to maintain their tenancies at the time of the crisis intervention and whose presence in community housing was stabilized. The outcome measurement will be targeted for 75% or higher. The proposal plans to focus on households facing many challenges with coordinated case management to prevent them from entering the emergency shelter system (which is beyond maximum capacity now with hotel/ motel placements continuing to be made) When ongoing relationships with the households can be maintained, data will be  kept on the numbers of people served, referrals made, what referral connections occurred, outcomes of the referral ( for example, did the person benefit from the job training, i.e. find a job, benefit from the domestic violence counseling as in taking out a restraining order, learn to balance their check book from a financial literacy course, pay their rent on time from a rep payee assignment).
