Dear Mr. Huntley:
Enclosed are two copies of the payment requisition as prepared by Dietz construction, and approved by
Huntley Associates, P.C. Please review and approve by endorsing below, and foaward this document to
the Office of Planning and Development, for approval of the reduction of the letter of credit.
The current application for payment, as detailed ui the enclosure, is for the sum of $22,300.00. This
amount represents the value of the work completed, and the materials stored on site as of the date of the
letter above. The request for reduction in the Letter of Credit is equal to tl~e amount of the payment
request plus. the 1.5% contingency, which is $25,645.00.
As the project has shown significant progress, we are also requesting a. reduction in the letter of 'credit
for the budget reserved for construction engineering services complete to date, broken out as follows:
Construction Layout: $13,000.00 * 100% complete = $13,000.00
On-Site Construction Supervision: $50,000.00 * 75% complete = $37,500.00
Total Letter of Credit Reduction: $50,500.00 * 1.15% (Contingency) = $58,075.00
