Pursuant to this statute,an employee is defined as"...every person in the service of another under any contract of hire, express or implied, oral or written." An employer is defined as"an individual, partnership, association, corporation or other legal entity, or any two or more of the foregoing engaged in a joint enterprise, and including the legal representatives of a deceased employer, or the receiver or trustee of an individual, partnership, association or other legal entity, employing employees. However the owner of a dwelling house having not more than three apartments and who resides therein, or the occupant of the dwelling house of another who employs persons to do maintenance, construction or repair work on such dwelling house or on the grounds or building appurtenant thereto shall not because of such employment be deemed to be an employer." MGL chapter 152, §25C(6)also states that"every state or local licensing agency shall withhold the issuance or renewal of a license or permit to operate a business or to construct buildings in the commonwealth for any applicant who has not produced acceptable evidence of compliance with the insurance coverage required." Additionally, MGL chapter 152, §25C(7)states"Neither the commonwealth nor any of its political subdivisions shall enter into any contract for the performance of public work until acceptable evidence of compliance with the insurance requirements of this chapter have been presented to the contracting authority." Applicants Please fill out the workers' compensation affidavit completely, by checking the boxes that apply to your situation and, if necessary, supply sub-contractor(s) name(s), address(es)and phone number(s) along with their certificate(s) of insurance. The sediment control method(s)implemented on each project vary depending on site-specific conditions, but may include one or more of the following measures: • Mud Processor-Equipment used to manage and process drilling spoils by separating solid and fluid materials removed from the borehole. • Containers-Drilling spoils are typically discharged directly into containers such as heavy-duty woven polypropylene sacks, impermeable 3-ply plastic pools, geotextile filter bags or lined roll-off containers during drilling. • Stockpile Methods-In some instances, drilling spoils or excavated materials from trenching may be temporarily stockpiled and contained using silt fences and/or straw bale dikes to prevent runoff. Excavated material is typically returned to the trench on the same day. • Dust Control-Water is sprayed to wet drilling spoils or excavated materials for dust control as needed. • Trailering-when necessary, tracked rigs will be trailered on and off private property to limit risk of street contamination • Diverter and Dumpster-Some specific drill rigs use a diverter to direct all drill cuttings into a dumpster via a hose.See the attached letter for details and photos of this system. These practices are used to ensure that the highest level of site and environmental protection are achieved as a normal part of system installation. The installation of above grade piping in an insulated enclosure according to our Design &Installation Standards will provide the necessary protection such that it will have the same performance and service life expectancy of a standard installation. The minimum allowable burial depth for insulated header piping shall be 12 inches below grade. o Whenever the header burial depth is less than 4 ft,freeze protection to 15°F is required. o To achieve a 15F freeze protection value,25%propylene glycol shall be used. • Propylene glycol is added to the system as part of the flushing and purging process during the heat pump installation phase of the project. Using the Kusudah-Achenbach equation published in IGSHPAs Residential and Light Commercial Design and Installation Manual to predict soil temperature as a function of soil type, location, time of year and burial depth, it can be shown that the soil temperature at a 1 ft burial depth will not fall below 25-30°F in this region. The requirement to freeze protect with 25%propylene glycol will provide a 10-15°F buffer between the minimum expected soil temperature and the freeze point of the circulating fluid. In order to protect the header piping from damage due to work by other trades, Dandelion requires the installation of warning tape and tracer wire in the trench.Additionally, each customer is given a scaled and dimensioned as-built drawing that shows the location of the borehole(s)and header trench on the property. These measures help to locate and identify the ground loop piping when necessary. As long as the system is installed in accordance with the requirements above, it will perform according to our design specifications and will not be at risk of damage due to freezing or damage. No.55865 t A ` 8/22/22 o g/• c.STEaE ../.4,, O333 North Bedford Road Suite 220 Mount Kisco,NY 10549 DANDELION October 19,2022 RE: Leak Detection&Emergency Shutoff The purpose of this memo is to address the requirements for geothermal heat pump system installation in the state of Massachusetts, specifically Section 9.2,Leak Detection,Emergency Shut-Offs,and Makeup Fluid in the DEP's Guidelines for Ground Source Heat Pump Wells, which states: The GSHP system shall have an automatic shutdown device(s)to minimize refrigerant, antifreeze or oil leaks in the event of a pressure/fluid loss. The shutdown mechanism shall be such that the system can only be reactivated by the GSHP system vendor or other qualified service representative that is qualified to detect and fix(if possible)the leak and conduct a post repair pressurization test. The GSHP that has been shut down due to fluid/pressure loss shall not be reactivated until the final pressurization testing described below in Section 9.6 has successfully been completed. The ability to reactivate the GSHP system that has been shut down due to a pressure/fluid loss shall not be provided to unqualified persons, including the home/building owners/occupants.A makeup fluid tank for either closed-loop or DX GSHP systems shall only be operated manually. The concern is that an automatic makeup fluid injection system could potentially result in the prevention of the activation of the automatic shutdown device in the event of a system fluid leak, thereby resulting in the release of additional refrigerant or antifreeze. To meet the provisions of this requirement,Dandelion: • Exclusively installs closed-loop systems using methods and materials that comply with ANSI/CSA/IGSHPA C448.0-16. • Performs a leak test according to ASTM F2164 to ensure that no leaks are present in the system prior to startup. • Requires the use of pressurized flow centers in our systems, which will not produce flow through the ground loop in the event of a leak. • Requires the installation of geothermal heat pumps that include a lockout alarm(s)in the sequence of operations to disable the compressor and circulating pumps in the event of loss of flow through the ground loop. • Does not install or make provisions for makeup fluid devices in our system. Therefore, when installed according to our Design and Installation Standards, all systems installed by Dandelion meet the requirements of Section 9.2 Leak Detection,Emergency Shut-Offs,and Makeup Fluid. System Loads System Loads or Peak Loads are calculated based on a variety of details for an individual residence. Assumed occupancy levels, the number of appliances operating, the number of doors & windows and the tightness of the construction all contribute to the amount of energy required to maintain the thermostat set points given the historical extreme weather conditions in your area. The peak loads used in this report were provided as listed in the following table. 1 kBtu/hr= 1,000 Btu/hr Total Total Heating Load Cooling Load Zone (kBtu/hr) (kBtu/hr) Zone SHF S1: Basement and Main Floor 39.11 23.34 0.940 S2: 2nd and 3rd 28.48 20.95 0.920 Total 67.59 44.30 1 of 9 , Massachusetts Equipment Schedule Based on the provided loads, the recommended heat pump schedule for this system is as follows: I ow Cap 1 kBtu/hr= 1,000 Btu/hr Heat ' Cool ' Water 2 Air' Cap.
