RE: MA5106S—115 and 123 Elm Street
Louis,
New Cingular Wireless/ ATT Mobility would like to place a Wireless Communications Facility inside of tow buildings owned by Smith College on Elm Street.
In the Helen Hills Hills Chapel 123 Elm Street, AT&T proposes to place 6 antennas and related equipment inside the steeple. We proposed to replace sections of the existing wood steeple
with matching fiberglass panels so the antennas which are mounted within the steeple can shoot through the fiberglass.
These antennas are connected with fiber and DC power cables to an equipment room which is in the basement of 115 Elm street which will hold the ground equipment. AT&T will place 2 HVAC
units outside of 115 Elm Street.
Enclosed please find:
Completed Building Permit Application
Planning board approval
Historical Approval
Contractors License
Workmen’s Comp insurance
General Liability insurance
Construction Control Affidavit
set of 11x17 plans
Structural and proposed modifications
Check for the fees
Disk with pertinent info including plans
Please let me know if there is any additional information that you need in order to process our building permit request. If everything is in order can you please return the permit to
me in the enclosed envelope?
Thank you in advance for your help,
