Perc Tests and Soil Evaluations The Board must provide a knowledgeable representative to witness all pere tests and soil evaluations. That individual should compile a log of the test data including Date, Address, Engineer/Sanitarian performing the tests and the following: • Soil Evaluation: Test pit #'s, a sketch of the lot, location of the test pits on the lot, depthof test pits, depth of the soil horizons, depth of weeping/standing water and estimated seasonal high groundwater. • Pere to : Test #, depth of peres, depth/time measurements, final perc rates lnspcq&n of the Installations of New Septjc Sy§tem and R ai s d A knowledgeable representative of the Board must inspect all new septic system installations and repairs to confirm compliance with the approved plans. The installation can only be buried after being viewed by both the system designer and the representative of the Board. The designer must subsequently submit a copy of an "as -built" plan to the board of Health. InvestigLtion of Complaints about Failed Septic Sysems and related Nuisances A representative of the board must investigate nuisance complaints relating septic systems and/or system failures and, when confirmed, issue orders to correct the problems under authority of State Law and Title 5. office Activities 1. ApRlication/Plan Review A knowledgeable representative of the board must review all applications and design plans to repair, upgrade or construct septic systems to determine compliance with the requirement s of Title 5. The following are among issues which must be addressed: inimum of 2 sets of plans with the application for new septic system construction; • The applicant must submit a m one approved copy of the plans is for the Board of health records the other is submitted to the building Inspector. A third copy is required if the applicant wants an approved copy. For repairs of existing systems one copy of the plan is acceptable. • The system installer listed on the application must hold a current installer's license in Northampton. • There are numerous items (see plan review checklist attached) which must be included in the septic design plan. If the proposed design meets the requirements of Title 5 the permit is issued upon receipt of the permit fee of $150.00, o the application and the permit (bottom portion of the application) is signed and A permit number is assigned t sent to applicant along with any approved copies of the design plan. A copy of the permit and signed plan is sent to the Building Inspector. • Pertinent information about the application is logged into the computer based Septic Permit records. The applications & plans are filed (by street #) in the active Title.5 filing cabinet near the front counter. 2. System A royal following inspections of Se tic System Installations M Upon receipt of the sign off and "as -built" plan from the designer of a newly installed septic system the Board representative completes a certificate of compliance (the middle portion of the permit application" and sends a copy to the owner of the septic system. For new construction, a copy of the certificate is also sent to the Building Inspector. The computer file is updated to show the dates of installation inspection by the Board of Health, the system designer and the date when the Certificate of Compliance was issued. The file for the approved septic systems is then filed, by street number, in the completed Title 5 files located in the Director's Office. Title s Inspection Reports • Title 5 Inspection reports (submitted to the Board of health by the Title 5 Inspector) must be reviewed to determine the status of the septic system having been inspected. • An Inspection report listing a system as "Passed" is recorded by adding the pertinent information into the computer based Title 5 Inspection report record and filing the report in the Title 5 Inspection Report in the Active File out front. • An Inspector report listing a system as "Failed" requires that the owner be issued a written orde to repair or upgrade the faled system within either, go Days (for a failed system which poses an immediate public health threat) or within 2 years (for a failure bases on other Title,5 violations, e.g. it extends below seasonal high groundwater). The order is sent certified mail, return receipt requested. The report and copy of the written order are filed by street # in the active file. • An Inspection report listed as system as "Re uirin further revi w b the B and of Heath" results in the Saord representative reviewing the data in the report, contacting the property owner, and determining the best course of action required either declare that system "passed" or issuing an order to repair or upgrade as might be required. Local i ipgrade ApprQvaI • When permit applications include a request for a Local Upgrade approval (LUA) it must be signed by the Board Health's Certified Soil evaluator who witnessed the perc test/soil evaluation. The soil evaluator either, in writing or, in person recommends issuance of the LUA to the board of health. After the Board of Health votes, during a posted meeting, to approve the requested LUA the sewage repair permit can be issued. • The permit is then issued as listed above with the addition of the signed Local Upgrade approval form being sent to the system owner. 5. Miscellaneous • From time to time the Board will receive requests for information about various septic systems in the City. To the extent possible, the completed files (listed by street#) are searched for pertinent information regarding the specific septic system. Current Title 5 Regulations went into effect in 1995 with minor revisions in the early 2000's. systems installed after 1995 should be readily available and useful. The more recent the system installation the more likely that there will be up-to-date information with plans and full descriptions. • For systems between 1977 and 1945 there will likely have limited information, most will also be available. • For systems constructed prior to 1977 there will most only be only a single page permit with no plans.
