Punch list items being completed. 21.1 7/10/96 -Marc asked floyd to assemble all extra stock into one location in a storage area. Item cont. 7/17/96 - Extra stock has been left in storage loft. Item closed. 21.2 7/10/96 - Marc asked floyd to compile and submit record drawings, warranties and maintenance manuals, as specified. Item cont. 7/17/96-No new info. Item cont. 21.3 7/10/96 - Marc asked floyd to have HVAC and electrical Contractors provide operating and maintenance instruction to the Bank's personnel , as specified. Item cont. 7/17/96- No new info. Item cont. 21.4 7/10/96- Marc asked floyd for a copy of the Certificate of Occupancy when he receives it. Item cont. 7/17/96 - Not yet received. Item cont. 21.5 7/10/96 - Marc asked Floyd to clean any windows that were not already professionally cleaned or still needed cleaning. Item cont. 7/17/96 -Work was completed. Item closed. 21.6 7/10/96 - Barbara Wallace requested SC provide costs for additional counter in Conference Room. SC will provide costs. Item cont. 7/17/96 - Barbara W. also requested an additional counter for area behind low teller counter. Costs submitted. Approval given. Item closed. 21.7 7/10/96 - Marc will call pre-cast Co. to try to expedite delivery of caps. Marc noted that the mason must provide temporary caps by Monday until the specified caps arrive. Item cont. 7/17/96 - Mason has provided temporary caps for opening. Awaiting correct caps. Item cont. 21.8 7/10/96 - Floyd was told by roofer that the roofmg mfr. prefers the method used over the membrane boots specified. Marc will check with mfr. Item cont. 7/17/96- No new info. Item cont. 21.9 7/10/96 - Marc had questions regarding the change order request submitted by Quantum Electric. Marc will ask Ziter to get Ray and meet with him. Item cont. 7/17/96 - Ray agreed to withdraw Quantum Change Order Request#004 due to the teller counter conduit never being installed and Marc agreeing to not ask for the call-for-aid lights over the bathrooms unless they are required by code. Item closed. 22.1 7/17/96 - Ray will have electrician train bank personnel in the use of the clock mechanism tomorrow. Item closed. 22.2 7/17/96 - Fred B. will talk directly to Rob Ricard regarding the balancing of the mechanical system. Item cont. 22.3 7/17/96 - Quantum submitted a Change Order Request for the installation of the ink sensor in the vestibule ceiling as requested by Owner. Item cont. 22.4 7/17/96 - Owner has requested closers on the three security doors. Marc will spec., Ziter will submit cost. Most signage installed. Clock installed. 15.1 5/22/96- Ziter was told that the Mosler teller window and night depository have been held up. They will not arrive at the site on schedule. Item cont. 5/29/96 -Mosler equipment still not on site. Item cont. 6/5/96 - Equipment has not yet arrived. Installation details were discussed. Marc would approve an inset installation in lieu of a standard external flange due to the irregularity of the masonry face. Floyd will work with Mosler re: details. Item cont. 6/12/96 - All Mosler equipment has been installed with the exception of the cash drawer under the drive- thru teller which has not yet been delivered. Item cont. 6/19/96 - Cash drawer installed. Gap created at counter/ cash drawer joint where Mosler does not supply trim. Marc will get Floyd info on black vinyl corners to use. Item cont. 6/24/96 - Floyd has not yet received the vinyl corners. Ziter will check on them. Mosler will supply a metal front panel next to teller window drawer. Ziter told him the following: -Counters will be in around Monday, June 10. -Cabinets can be brought in and placed at any time and painted early/middle of next week. -Bank should be substantially complete Monday, June 24. Item cont. 6/12/96 - Revised schedule as follows: -Counters are in Wednesday, June 12. -Cabinets can be painted Thursday of next week. -Bank should be substantially complete and ready to open Monday, July 4. Marc stressed that the bank must be able to be open on this day, without fail. Item cont. 6/19/96 - Ziter's revised schedule: -Interior complete by July 1. -Exterior substantially complete by July 15. -Bank can definitely open for business July 15. Item closed. 20.5 6/24/96 - Marc was assured by tile installer and by Bob King that the tile in the vestibules would be installed to be even with the entry carpet. Item cont. 7/3/96 - Tile installed. Some tiles will be noted for re-installation in the punch list. Item closed. 20.6 6/24/96 - Bathroom door hardware was supplied and installed backwards. Chrome finish should be facing the interior of the bathroom only, as is typical. The side facing the break room should match the same finish used throughout the rest of the bank(including the hinges). Item cont. 7/3/96 - Items repaired. Item closed. 20.7 6/24/96 - The Store 24 sign was found to be slightly leaning to one side. Upon further investigation, it was found that the sign had a minimal foundation barely sufficient to keep the sign standing. Marc will notify Barbara immediately as to the existing condition and the possible options. Item cont. 7/3/96 - The existing sign foundation was cut back slightly, the new curbing was installed a couple inches farther away from the sign than noted and a concrete island was poured to help stabilize the sign. Additional costs will be issued. Item closed. 20.8 6/24/96 - Marc asked Ziter and Floyd to ensure that the blinds in the conference room not be installed right away to see if they need to be installed at all. Masonry continues on exterior. 9.5 4/10/96 - Rob noted that there was never a legal sanitary sewer hook-up to the existing building. Peter McNulty of the Northampton DPW told him that we will have to connect directly to the main sewer line across King Street. DPW assumes no responsibility for costs. Item cont. 4/17/96 - Rob noted that our sidewalks and curbing will be held up until this issue is resolved. Item ' cont. 4/24/96 - Barbara asked Marc to hold off on giving Bill Canon the go-ahead to produce drawings needed for sewer hook-up until she works things out with the property owner. Item cont. 5/1/96 - Ziter warned that delays in going ahead with sanitary sewer line work might delay entire project. Marc will call Tim Crimmins today to obtain a go-ahead for Bill Canon's services to allow us to get DPW approval ASAP. Item cont. 5/8/96- Bill Canon has been given the go-ahead. Drawings will be submitted soon. Item cont. 5/15/96 - Drawings submitted to SC. Rob O. submitted them to DPW. DPW has not yet issued permit. Rob O. has issued verbal price. Item cont. 5/22/96 - Marc, Ziter and Rob accompanied Barbara to the office of Denis Perlman's lawyer, Tom Lesser to discuss the lack of proper sanitary sewer connections to the site. Item cont. 5/29/96 - Barbara has give Ziter the go-ahead to proceed with the work. Rob noted that the Northampton Police will not allow him to leave metal plates on road or reduce parking spaces on King Street over the weekend. This means that Rob cannot begin work until Monday. Still awaiting construction permit. - Barbara asked that special care be given regarding safety of VA patrons during the construction process. - Ziter hopes to begin foundation work as soon as weather allows. He will be using insulating blankets, hay and plastic to keep concrete warm and dry while curing. -Job meetings will be held on Wednesdays at 8:30am beginning December 20th if possible. -Marc will finalize sign specifics with Ziter soon. Barbara wants bank logo on sign. -Marc will talk with Don Armour re: changing light fixtures in break room and vault. - Barbara and Marc will work out cabinet needs in regards to the break room. - Ziter will coordinate placement of job trailer with Ray regarding best way to supply it with electric and phone. The following items were also discussed: 1. Provide list of sub-contractors: addresses, phone, fax, contact person. Also include owner, architect, engineers, bldg. inspectors, etc. Update as required during project. 2. Provide Schedule of Values: break down as much as possible (rough from finish carpentry; trusses delivered from trusses installed, hanging GWB from finishing, floor from floor, etc.). 0 . �. 3. AIA forms to be used: Change Orders Stored Materials on site Stored materials off site Requisition 4. Requisition: pencil copy at least one week before final - fax to me or give to me at meeting before final. 5. Construction Schedule (bar chart) - Submit one at very beginning of job and include updated one with each requisition. Schedule to indicate progress of work stages, area separations, and service interruptions. 6. Shop drawings: a. GC to thoroughly review before submitting to me; if GC rejects them return them to sub and have them resubmit before sending on to me. b. Shop drawings must have GC's stamp of review and GC's comments. Provide transmittals. c. Make sure all appropriate trades have reviewed shop drawings and coordinated with each other and GC. d. Ductwork layouts are required - coordinate with all subs involved (plumbing, electrical, ceiling tile, etc.). e. Give all shop drawings to me -I'll distribute to engineers. f. Give me at least 2 weeks to review. g. If submitting a lot of shop drawings give me a priority list of what to review first. h. Large item shop drawings (such as roof and floor trusses) must be submitted all at one time - not piecemeal. i. Do not, proceed without stamped shop dwgs - do at own risk. 7. Samples: give me actual samples and colors. Colors printed on paper will not be acceptable. 8. Substitutions: GC to give full backup or A/E won't look at - up to GC to show that it's equivalent or better. 9. Note proprietary specs - substitutions may not be allowed. 10. Change Orders: give full breakdown of materials, # hours worked and by whom, provide invoices. 11. GC responsible for coordination of all trades - make sure subs and suppliers look at all drawings and specs, not just those of their trade (eg. truss manufacturer needs to see all drawings not just framing drawings). 12. Remember - Sales tax is applicable on this job. 13. Specifications and drawings are complimentary- questions or conflicts please ask me. 14. Questions for engineers should go through me first. 15. Engineers will not come to every job meeting unless I'm notified ahead of time that they are needed. 16. Provide temporary heat as required for all stages of construction. ,- ,• ,`_ 17. GWB to be stored at least 48 hours in heated building before installation. 18. Building to be heated— days before and — after installation of GWB. 19. All materials to be stored properly and protected from weather. 20. Provide temporary protection (poly sheeting, etc.) of elements under construction. 21. Owner is responsible for testing (unless otherwise specified) but GC must coordinate and inform owner well in advance of when testing is needed, especially where time is of the essence (soil compaction, concrete, etc.). Additionally, allow enough time so that Architect can review results and consult with consultants. Architect will not allow work to proceed without testing. 22. Provide mock-ups for Architect to review and punch where applicable. GC to use this mock-up and punchlist for basis of quality and level of finishes of other units. 23. Remember that the site is downtown -secure all areas during construction and nights and weekends, provide good fencing and clearly mark all areas which may constitute a danger to pedestrians- clean up, keep the dust level down as much as possible, etc. 24. Make sure Owner has letter from abutter allowing work on their property before working past the property line. Be careful about excess runoff, etc. onto adjacent property. 25. Be aware of security issues. 26. Keep in GC office: plans and specs, approved shop drawings, change orders, supplemental instructions and sketches, etc. 27. GC to maintain and continually update as-built drawings and specs. Keep as-builts up to date for submittal to Owner at end of job. Keep extra copies of appropriate shop drawings, product data, etc. to submit to Owner with rest of maintenance information needed at end of job. To the best of my knowledge and belief, these meeting notes are considered to be a true and accurate record of all items discussed.
