Section 110.R5.1.2
Homeowner: Person (s) who own a parcel of land on which he/she resides or intends to reside,
on which there is, or is intended to be, a one or two family dwelling, attached or detached
structures accessory to such use and/or farm structures. A person who constructs more than one
home in a two-year period shall not be considered a homeowner.
Section 110.R5.1.3.1
Any homeowner performing work for which a building permit is required shall be exempt from
the licensing provisions of 780 CMR 110.R5, provided that if a homeowner engages a person(s)
for hire to do such work, then such homeowner shall act as supervisor.
Such homeowner shall submit to the Building Official, on a form acceptable to the Building
Official, that he/she shall be responsible for all such work performed under the building permit.
As acting Construction Supervisor your presence on the job site will be required from time to
time, during and upon completion of the work for which this permit is issued.
Also be advised that with reference to Chapter 152 (Workers' Compensation) and Chapter 153
(Liability of Employers to Employees for injuries not resulting in Death) of the Massachusetts
General Laws Annotated, you may be liable for person(s) you hire to perform work for you

applicant who has not produced acceptable evidence of compliance with the insurance coverage required."
Additionally,MGL chapter 152, §25C(7)states"Neither the commonwealth nor any of its political subdivisions shall
enter into any contract for the performance of public work until acceptable evidence of compliance with the insurance
requirements of this chapter have been presented to the contracting authority."

Labor and materials for following:
Strip and dispose of existing shingles on main house
Install ice and water barrier along eaves and valleys and install synthetic underlayment
on remainder of roof.
Install new drip and rake edge and 35 year architectural style shingle color to be
determined.
Install new ridge vent and cap shingles.
Base price $17,650
* Note, Inspection of skylights to determine sealant integrity and glass at which time a
replacement may be required. Customer will be notified of approximate cost to replace
and shall be billed separately from the above scope of work at the labor rate of$100 per
manhour plus material.
Terms
A $5883.33 deposit is required and signature of acceptance of above scope of work.
Second $5883.33 cash disbursement upon removal of existing asphalt shingles and
installation of ice and water barrier and synthetic underlayment.
Final payment of$5883.33 will be due on day of completion of roof as outlined in the
above scope of work.
Please note any additional work outside the above scope of work: shall be billed as time
plus material based on $100 per man-hour and will be billed separately from original
scope of work. In the event that the terms are not met as outlined the owner shall be
subject to a finance charge including but not limited to 1.5 percent monthly of the
balance owed the contractor. The owner will be responsible for all charges related to the
collection of any outstanding balances and all attorneys fees related to the collection of
the balance.
