Dear Commission Members:
The project to reconstruct the track and softball field at Smith College has been
completed. All work and site improvements have been done in substantial
compliance with the approved plans. During construction, The Berkshire Design
Group was on site to review progress and coordinate minor changes as was required
due to site conditions. The contractor has completed all work and the area is
stabilized and vegetated.
Attached please find WPA Form 8A-Request for Certificate of Compliance and an
as-built survey for the Track & Softball Reconstruction Project.

subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
