49 #604 Complaints 2009-2014 TENANCY PRESERVATION PROGRAM OF THE MENTAL HEALTH ASSOCIATION 15 GOTHIC STREET NORTHAMPTON, MA 01060 413.584.2003 Outline of Doris Purrington case TPP had an open case with Ms. Purrington in 2005, due to hoarding, sanitation violations and Alcohol abuse. At that time, TPP recommended detox, followed by some ongoing addictions counseling. Ms. Purrington ultimately refused any treatments and TPP closed case. 2/13/2009: TPP asked, by Northampton Housing Authority (NHA) to attend inspection with Board of Health (BOH) and Northampton Fire Dept (NFD) at Doris Purrington's apartment. When all parties arrived, Doris could not open her door due to debris piled up near door. NFD pushed the door open, which was being blocked by piles of newspapers. Ms. Purrington was intoxicated. NFD and BOH inspected entire apartment to find urine soaked debris, vomit and rotting food. The unit had strong odor of urine. After inspection, Elder Services was called, as we had an elder at risk of having unit condemned. Elder Services (HVES) requested that BOH not condemn unit, until they can put together a plan of action to get Ms. Purrington some help. 2/20/2009: Re-inspection scheduled by BOH. Ms. Purrington was sober, and present during time of inspection was a CHORE worker, secured by HVES to do heavy clean-out of unit. Significant progress had been made by CHORE, and recommendations were made to keep CHORE in place to continue progress. 2/27/2009 to 3/20/2009: Four inspections during this time, with Ms. Purrington and CHORE worker making significant progress. Team thinks one or two more inspections are all that is needed. Ms. Purrington still sober. 3/27/2009: Re-inspection from BOH and NHA. Schedule next inspection following week. 3/31/2009: Ms. Purrington began drinking on or before 3/30, and is sent by ambulance to ER Dept due to heavy intoxication. Released later that evening to her apartment. NHA, BOH, HVES and TPP are all in communication throughout the week, to come up with a plan to get Ms. Purrington into treatment. 4/3/2009: Inspection at Ms. Purrington's, with knowledge that she has been drinking for at least 5 days. Ms. Purrington is heavily intoxicated, with 2/3 of a1/2 gallon vodka bottle at her side. BOH takes pictures of a unit that was in great shape only a week ago. Now, unit is condemnable. All parties meet downstairs to discuss a plan that will ultimately steer Ms. Purrington into treatment. BOH condemns unit, and NHA calls Police and ambulance to remove Ms. Purrington to ER. Team goes up with Emergency Responders, and Ms. Purrington has consumed another 1/3 of bottle of vodka in the 45 minutes it has taken to write condemnation order, and call in emergency responders. Ms. Purrington is taken to Cooley Dickinson Hospital, and BOH and Police inform Hospital that Ms. Purrington cannot return to unit as it is condemned. Ms. Purrington admitted to medical floor. 4/7/09: Planning meeting for discharge held at hospital, where Soc. Workers from CDH, BOH, HVES, and TPP were all present, as well as Ms. Purrington's sister on a conference call. The entire team supports a residential detox/treatment program for Ms. Purrington, followed by an outpatient program. CDH social workers have already made calls to treatment centers, and it is a possibility to get Ms. Purrington into HOPE Center in Spfld. CDH will hold Ms. Purrington as long as possible, and transfer into Nursing Home while she awaits placement into a Treatment facility. 4/10/2009: Ms. Purrington transferred into Calvin Coolidge Nursing Home, to await placement into treatment facility. Intake calls are made to Hope Center, and TB test administered. Ms. Purrington is in total compliance with the plan. In the meantime, Ms. Purrington's sister and friend are trying to clean up unit so that Ms. Purrington can return when she completes treatment. 4/16/09: Ms. Purrington admitted to the Hope Center from Cal Coolidge NH. TPP transports her to Springfield for admission. 4/29/2009: Ms. Purrington d/c'd from Hope Center to apartment, which had been inspected before her arrival, and BOH allowed re-entry. 4/30/2009: Ms. Purrington completed intake at an IOP in Florence, and will begin the START program 5/1/09. 5/18/2009: Court agreement entered into. 5/22/2009: Ms. Purrington graduates from START program. 6/14/2009: Ms. Purrington begins drinking binge again, according to NHA. BOH informed, and plan is to inspect, and condemn unit again if necessary. 6/29/2009: Ms. Purrington has stopped drinking again, and according to NHA, apartment never got bad enough to condemn, due to the heavy clean-out that had occurred. Reminded Ms. Purrington to stick with other terms of the agreement in which she is suppose to hire CHORE worker for 2hrs/month, to do cleaning. 8/18/2009: Ms. Purrington is drinking again, on her birthday. 8/28/2009: Per court agreement, BOH and NHA in to inspect with TPP present. Unit is in disarray, with foul odor on urine throughout. Papers piling up near door and thru unit. Vomit is on floor. BOH had noted that Ms. Purrington came thru their offices with feces on her pants earlier this week. Ms. Purrington does not seem intoxicated during inspection, and is clothed. BOH, NHA and TPP discuss whether it is necessary to condemn unit today, or wait until Monday's court appearance to have Ms. Purrington in front of the court.
