The Northampton Board of Health is acting under the authority of
MASSACHUSETTS GENERAL LAWS, 105 CMR 410.000: State Sanitary Code Chapter II:
Minimum Standards for Human Habitation. You are hereby ordered to make a good faith
effort to comply with the formal order issued to you above within the time frame
listed. Failure to do so could result in legal action taken against you to pursue compliance
with this order.
A re-inspection will be taking place on or around May 11, 2009. If you would like to
be present at this inspection please feel free to contact me at the Board of Health and we
can find a time that is mutually agreeable. Failure to comply with any order issued pursuant
to the provisions of 105 CMR 410.000-shall upon conviction be fined not less than $10 or
more than $500. Each day's failure to comply shall constitute separate violations.
Please be advised that if you are aggrieved with this order you have the right to request a
hearing. You must submit a written petition requesting a hearing within seven (7)days from the day the
order was served

Enclosed is the estimate for the replacement/repair of the windows at Millbank Place If 7.
The parts have been ordered,but they are not items that can be picked up in a shop. They
have to be manufactured and will take weeks to come in.
The skylight glass and the window glass have to be replaced simultaneously as they are
all one piece connected together. There isn't any way to replace the windows without
replacing the roof glass.
I am, therefore,requesting an extension of the time-frame for the work to be completed.
A&Z Construction has agreed to install all when the parts arrive. As stated,this will
require a lift for the work to be done.
in the meantime, I will ask A&Z to do what they can to get the window to close securely.
If this was a regular or routine repair,the work would be complete- I try to take care of
our residents and buildings;sometimes the problems axe bigger that expected.
l would appreciate whatever assistance you could provide to grant an extension that
would coincide with the time-frame of the A&Z work. 1 can be reached at 582-9970

Re: Construction project
Beginning on Monday,July 13,2009, extensive construction to replace the skylight and
windows over the right-side front entrance to the building will begin. A&Z Construction
will park a lift in a few parking spaces near the big tree. Beginning at 7:00 am on July
13`a, please do not park in that area.
The current skylight and windows will be removed. A new skylight and windows will be
installed. The small roof will be replaced as well.
To the residents in units# 1 and#7, please move all personal property out of the window
area, and remove all items from the walls in the window area, and adjacent to it as well.
That area should be completely empty by 7:00 am on Monday, July 13`h. Since there will
be substantial hammering and sawing,the walls will vibrate. To be on the safe side,
anything of value that you have on any walls inside your unit should be removed.
The offices of Jekanowski and O'Neil and Robert Ostberg, will be affected as well. It is
unlikely that personal property in your offices would be affected by this work,but taking
the steps to safeguard any valuables, such as wall art, cannot hurt. Depending on the
location of the lift,your clients may be unable to use the sidewalk leading to your office.
We will know better once the lift is in place and the work has commenced.
