Dn December 22, 2003, a representative from our office performed an inspection of the
septic system repair installation referenced above. The system was installed by Duffy
Nillard of Northampton, MA.
Dur representative found that the system appears to have been installed properly and in
jeneral accordance with our system plan dated 08-15-03. The as-built locations of
'elevant system components have been documented on the attached sketch.

RESTRICTION
I, Thomas Pascucci, owner of the real estate known and designated as
349 Coles Meadow Rd. Northampton, Hampshire County, Massachusetts by virtue
of a deed dated February 28, 1997 from James and Jacqueline Engel and legally
described in The Hampshire County Registry of Deeds book 5069, page 252, hereby
impose the following permanent restriction upon said premises, into perpetuity:
Due to the design of the septic system, a garbage disposal will never be
installed on the premises.
This restriction is applied to the subject premises in compliance with the Waiver of
Garbage Disposal/Sewer Capacity Requirement issued by the City of Northampton
Office of the Board of Health dated May 29, 2003
