- 7:10pm -Responded to a call from Safety Dispatch that Building #4 has a sewer backup in
the building. Currently present at the site was Tony Patilla,Bldg. Commissioner, Duane
Nichols and Dana Chevron,Fire Dept., and Jackie Milian, Complex/Site Mgr.
- 9:15pm-Captain Devine (cell 210-7596) from Fire Department reported sewage back-up
on the first floor of Building#5 and as a result that all tenants on the first floor to be
evacuated of which they were.
- 10:00am-Received call from dispatch suggesting that the tenants in entire Building#4R
and in all units to be evacuated of which they were.
- 10:10am-Received call from Captain Devine that sewage was going into Building#4R and
they are evacuating the tenants of the first floor basement apartments.
- 12:00pm-Arrived at the Shelter at which place the tenants were at which was the Florence
Community Center on Pine Street. Approximately 40 tenants were being housed there. Some
tenants were being housed at the Florence Grammar School on Park Street.
In building#5 only first floor was evacuating. The building is being monitored for methane
gas by the Fire Department
- 1:30pm-Updated Duane Nichols on how things were being set up at the school

- 9:30am-Retuned for inspections with Duane Nichols, Tony Patillo and Jackie Millar.
Conducted a follow up inspection of the apartments with Tony Patillo, Building Inspector,
and Duane Nichols, Fire Department, and Jackie Milian, Grounds Manager., All apartments
are still in the process of being cleaned by Service Clean Master Company. Company owner,
Donald Anderson was also present at this time.

- 2:OOpm-Received call from Jackie that all of the units on the first floors of both buildings
had been de-odorized, sanitized, and all water has been vacuumed up. I verified this Mr.
Anderson from Clean master and ordered the floor to be barricaded off and granted
permission for the residents of the second and third floors to be allowed to occupy there
units.
September 19, 2005
- 3:55pm-Received complaint from Rose Guessing tenant of the 3rd floor Meadow Brook
Apartments. She is concerned about the blower machine that is in the hallway being used to
dry off the floors ion the 1"floor(Basement apartments) and questions the safety of the air to
breathe. After looking into the situation I called her back and left a message on her
answering machine informing her that a professional cleaning and restoration company de-
odorized and decontaminated all wet areas and wet items from the sewage back-up. I told
her that the machines are only air blowers that speed up drying on the first floor and that
there is contamination in the air.
September 20,2005
11:30am-Received a message from Jackie the manager of Meadowbrook that in Bldg. 4R,
units 16, 17, and 18 are ready for inspection and to call her to schedule inspections. The
walls, floors and furniture have sanitized, cleaned and deodorized as well as dried.
- 1:30pm-Also received a call from Joe Longtin,Property Foreman, stating that the
apartments in 4R are all clean and ready for re-inspection to allow re-occupancy.
- 2:30pm -Received a message from Corrine from the Mayor's office to report
that Sherry Long had called to express her thanks and appreciation for the
inspections and the monitoring of Meadowbrook by the Board of Health, Fire
Dept and The Building Dept. since the sewage back-up. She also wanted to
thank everyone for communicating effectively with Jackie during this incident
and the clean-up.
September 21,2005
5:35pm-Conducted inspections of the units with Joe Longtin, Prop. Foreman and Andrell
Yarbrough, Assistant Manager to Ms. Millar.
Findings were:
1). In Building 4-R the hallway rug had been removed and walls cleaned and sanitized.
- 2). In apartment 418 there was sewage back-up only in the bathroom and the floor was
cleaned the wet rug was removed. The entire rug in the apartment was affected by sewage
but it was deodorized and shampooed.
- 3). In apartment #17 the sewage back up was confined to the bathroom and it was cleaned
and sanitized. The entire rug in the apartment was shampooed.
- 4). In apartment #16 the same findings were as observed in#17.
Note: Since the power was never turned off in Building 4-R and the refrigerator doors were
never opened and the temperature in all of them ranged from 38 -43 degrees the food stayed
wholesome and was allowed to be used at the discretion of the tenants.
- Approval was granted by this office to allow the tenants to re-occupy these
units.

day of September 21.

Received a call from Ned Huntley,DPW. He asked the following question: Did you or
anyone see sewage in the manhole at Meadowbrook on the evening of Sept. 15,2007, when
they had the sewage back-ups in the 2 Buildings on that evening. I responded by tell him that
I did not personally look into the manhole so that I could not answer that question. He said
that he would call the Fire Dept. and The Bldg. Dept..

Dear Mr. Mathieu,
Thank you for your visit to Meadowbrook Apartments. I want to assure you that the problem we
have in 2011 will be rectified. I have scheduled our contracted carpet cleaning company(Chem-
Dry)to come out on Tuesday March 14,2006 to shampoo the carpet in the dining and living
room area I will also at that time remove mold from the baseboard and wall by using Hydro.
Peroxide.
The on going problem in the boiler room area has been fixed. The problem was the release valve
and the back flow. Our maintenance technician John Lopez replaced the valve on 3/6/2006 and
the back flow was replaced on 3/7/2006 by our Plumbing contractor Lambert LI#9762. (Lambert
Plumbing&heating INC 413-547-0382). Since this problem has occurred our Maintenance Tech
has been checking the boiler room for water leakage every morning.

Received a call from a Mary Likins today about possible fire code violations at
Meadowbrook Apartments, located at 491 Bridge Road. She resides in building
28 unit 11. Specifically, mention was exit signs (lack of), landlord emergency
contact information and an electrical outlet that was described as surging. When
asked to define surging, she described that her refrigerator goes on and off
needing to be reset, an night light that flickers and vacuum cleaner that goes no
and off when operating from this outlet. When asked if there was any arcing or
sparks from the outlet, I was informed there was not. She is concerned this may
be result from some flooding that occurred a while ago.
I inspected building 28 about 1:30 pm on this date and was accompanied
property manager Andrell Yarbrough and security from the complex. I found the
building's common areas to be clean and orderly. Fire extinguishers were
compliant and properly located. A check of the fire alarm showed no signs of
trouble alarms or problems.
The issue of exit signs falls in under the state building code. I will forward a copy
of this report to the building commissioner for review. I have a work order from
management showing that they switched out a GFI outlet. Tenant stated that she
was still having problems. I will ask the Electrical Inspector contact management
discuss problem.
I spoke with Ernie Mathiew from the Board of Health in regards to the emergency
contact information. The Board of Health will address this in his report from his
inspection earlier this week.

Wall vent grate and vent duct has dust accumulation. The tenant and the tenant's Mother have taken
an air sample of the air coming out of the duct with a Petri Dish as instructed by the laboratory that
provided it to her. The dish shows growth of possible mold and bacteria cultures. Tenant and family
members claim to have severe allergies and allergy reactions to molds which they said can be
verified by their Primary Physician.
ORDER: Air sample and air testing must be taken from the bathroom air duct vent and analyzed for
mold cultures by a Certified Laboratory no later than December 8,2006. Copies must be
faxed to the Board of Health(413-587-1221) as soon as compiled by the Laboratory. If the
test is positive for mold you are hereby ordered to professionally treat all mold growth in the
vent system as well as clean the system by removing all the dust. Arrangements must be
made with the tenant by giving them sufficient notice prior to conducting any work in the
apartment,if needed. (See Section from Chapter 2 Code Below).

10: Access for Repairs and Alterations
Every occupant of a dwelling, dwelling unit, or rooming unit shall give the owner thereof, or his
agent or employees, upon reasonable notice, reasonable access, if possible by appointment, to the
dwelling, dwelling unit, or rooming unit for the purpose of making such repairs or alterations as
are necessary to effect compliance with the provisions of these minimum standards.

Received a call from a Mary Likins today about possible fire code violations at
Meadowbrook Apartments, located at 491 Bridge Road. She resides in building
28 unit 11. Specifically, mention was exit signs (lack of), landlord emergency
contact information and an electrical outlet that was described as surging. When
asked to define surging, she described that her refrigerator goes on and off
needing to be reset, an night light that flickers and vacuum cleaner that goes no
and off when operating from this outlet. When asked if there was any arcing or
sparks from the outlet, I was informed there was not. She is concerned this may
be result from some flooding that occurred a while ago.
I inspected building 28 about 1:30 pm on this date and was accompanied
property manager Andrell Yarbrough and security from the complex. I found the
building's common areas to be clean and orderly. Fire extinguishers were
compliant and properly located. A check of the fire alarm showed no signs of
trouble alarms or problems.
The issue of exit signs falls in under the state building code. I will forward a copy
of this report to the building commissioner for review. I have a work order from
management showing that they switched out a GFI outlet. Tenant stated that she
was still having problems. I will ask the Electrical Inspector contact management
discuss problem.
I spoke with Ernie Mathiew from the Board of Health in regards to the emergency
contact information. The Board of Health will address this in his report from his
inspection earlier this week.

Received a message on Sunday, April 9, 2006 at 4:48pm on the Board of Health voicemail from
Police Officer Capt. Clark that one of his officers's,Pablo Rodriquez responded to a call and went
to the residence of Ms. Lopez.He observed the apartment to be cluttered and messy as a result of
poor housekeeping. He observed 2 cat litter boxes full of feces and the room had a strong odor. He
stated the he was concerned because there were 2 children living in the apartment.
On the morning on Monday,April 10, 2006,I visited the apartment and observed the same findings
of Captain Clark. However,the cat litter boxes were still full and she emptied them while I was
there and filled them with clean litter. I told her she had to wash the floor to rid the urine odor. I
related to Ms. that she had to clean her apartment and take care after the cats. She expressed that she
came home from the hospital on March 18,2006, after having her newborn and was under Doctor's
care. She broke down into tears and stated the she is having some problems and was getting
assistance from DSS and had a home Health Aid. Her DSS case worker is Maureen Bennett at 413-
585-8075 and Noreen McEvady. I verified this with Ms.Bennett and she said she was scheduled to
meet with her on Thursday and that she would the oversee that cleaning of the apartment gets done
and also would contact her Home Health Care Worker in Greenfield whose phone number was 413-
775-5000 for her assistance in cleaning. I latter this day informed Officer Rodriquez of my visit and
he said that he would follow-up and visit her again and report back to me if there is need for further
action. He expressed some faith that there would be improvement since DDS is involved and they
have now been contacted by the City to take action.

To Whom It May Concern:
My name is Diana Lorenzo and I am writing this letter on behalf of my
grandmother Nelly Rios. She has been a tenant of the Northampton Housing
Authority for twenty years. In her twenty years residing at 81 Conz Street
Apt#515 she has paid her rent on time, abide by all the rules and regulations
and has not caused any problems to the NHA or to other tenants. In the last
few years repairs needed for her apartment have been reported and as of
this day those repairs have not been taken care of and other repairs have
been necessary.
In the last twenty years Mrs. Rios apartment has not been repainted and due
to the neglect of the NHA paint is peeling from her ceiling. This is not only a
concern for the cleanliness of her home but for her well being. The old paint
is falling on all areas of her apartment including her kitchen counters,
making it difficult for her to maintain a clean, safe and sanitary condition in
her apartment.
Another concern with Mrs. Rios apartment is the roaches. These pests have
been when present new tenant moved in nexxt door. It has been very difficult for her to
get rid of these pests considering that there coming from another apartment.
This is also making it very difficult for Mrs. Rios to maintain a clean and
sanitized home. I believe the best way to care for this situation is by
exterminating the entire building.
A few other concerns are that the stove belonging to her apartment is nly
50%functional. Two eyes work and two eyes are not working The
stove is owned by NHA and should be replaced as soon as possible. The
screen belonging to her room window is missing and preventing her from
opening the window to allow fresh air to enter her room without mosquitoes
and other bugs entering. This also prevents her from living in a clean, safe

and sanitary environment. The last concern is that the knob on her sliding
glass door is broken and is making it very difficult for her to open the door.
In order to comply with the standards set by the U.S Department of Housing
and Urban Development (HUD), apartments are to be painted on a regular
schedule. Alterations, redecoration and repairs to interior of apartments
and to appliances owned by the Authority are the sole responsibility of the
Housing Authority. I understand an offer was made for Mrs. Rios to move to
another apartment and she refused Mrs. Rios is 77 years old and will like to
remain in the same apartment she has been residing in for the last twenty
years. She's an elderly lady and there's no reason why she has move all her
belongings and relocate to another apartment, possibly another building in
order for NHA to do their job.
Elderly tenants often have no one advocating for them or any family member
to voice their concerns or look out for their well-being. In this case, I am
Mrs. Rios granddaughter and 1 am advocating for her. As you are aware
Mildred Jimenez has met with Maureen Cayer in which there were no
positive outcomes to these concerns. Mrs. Cayer suggested that Mrs.
Jimenez write a letter, which she did, requesting for attention to be focused
on Mrs. Rios apartment. Months have passed and still no positive outcomes.
This is the final attempt to have the NHA take responsibility and repair,
replace and exterminate Mrs. Rios apartment If these matters are not
resolved in the next 30 days I will be forced to bring my concerns to the
Northampton Board of Health.
I would like to thank you for your immediate attention to these matters and
hope that we can resolve this without having to involve higher authorities. I
am sure they will agree that these concerns are jeopardizing Mrs. Rios 's
well-being and her living environment. If you have any questions I can be
