Dear Ms. Alicea:
This letter is a reminder notice to you that you must pick up and pay$50.00 for the 2014 Lodging House
License#309 for The Lorraine,and pay$50.00 for the 2014 Lodging House License#310 for The
Elmhurst. Renewal documents for these licenses were mailed to you on November 1, 2013. You are now
operating without valid licenses.
As of February 1,2014,a late fee of$15.00 per month will be added to the cost of each of the
licenses.
I must submit the names of owners of all overdue licenses to the Northampton License Commission at the
March 5,2014 meeting. The Northampton License Commission reserves the right to take further action
on non-renewed licenses, which may include suspension or revocation of such licenses.

to be responding late to your voicemail of 2/20115. Could you fill me in about the status of this unit-NFD had concerns about Gutterlsqualor from
response to the leaking pipes recently;the pictures were not as compelling to us and I think I was told that HAP is working onlwith the occupant to
we the unit? It has been long enough that I may be confusing the bits of info I have received so far...please give me a picture of the situation.

NARRATIVE
Dispatched to 98 Pleasant St.for a reported water problem and possible ceiling collapse.Upon arrival,crew found water
coming through the ceiling at#98 Pleasant St.Investigation revealed water coming from the third floor Unit 311 affecting
the basement,first and second floors.Source of water was from a burst steam pipe supplying water to the radiator. Fire
alarm system was activated due to steam in hallway during the investigation.Water damage was limited to Units 311,211
and the business located at 98 Pleasant St.Further investigation of possible water damage in adjacent units showed
extremely hazardous conditions in Unit 312.Conditions induded but were not limited to burners operating on stove top
without supervision,heavy Butter and poor sanitary conditions. Building Inspector on scene and health department was
contacted for further direction.Conditions were noted and pictures were forwarded to Health Department.Building was
turned over to Building Inspector for further assessment as needed.Fire alarm system reset without incident.See files for
pictures and further information.
