Re: Status of the Septic System at 686 Westhampton Road

You have requested a statement from the Board of Health on the status of your septic system
at 686 Westhampton Road,Florence.
While a formal Title 5 Septic System Inspection has not been conducted, I have observed the
condition of your septic system and found that it is not in compliance with Title 5 of the State
Environmental Code.
On September 25, 1997 I witnessed a percolation test on your property, conducted by Dan
Nitzsche of Howard Environmental Services. During that test the existing septic system was
inspected and it was found that a pipe from the septic tank ran into the adjacent pond/wetland
area on your property. Such a condition constitutes a violation of Title 5 and in effect causes
your septic system to be listed as failed.
To date, a new Septic System Plan has been prepared and the Board of Health has issued
Septic Repair Permit. Once the new septic system has been installed the Board of Health will
inspection the new system and issue a Certificate of Compliance.

On September 30, 1998 as a representative of Howard Environmental Services I
inspected the septic system at 686 Westhampton Road (Route 66), at the property of Roxanna
Glynn The system was Stalled by John F. Tautznik&Sons, of Easthampton,MA.
I found the system to be installed in accordance with our approved septic system design
dated May 26, 1998. See the accompanying as-built sketch for location of the system
components.
This letter shall serve as Engineer and Installer certification that the system was installed in
accordance with Title 5 and our approved system design. If there are any questions, please
contact our office.
