The system was installed by Tom Childs of Westhampton, MA. Our representative found that the system is installed properly and in accordance with our septic plan dated 5-7-97. The as-built locations of all system components have be documented on the attached sketch. This letter shall serve as Engineer and Installer Certification that the system was installed in accordance with Title V and our approved system design. Dear Mr. Tacy: On August 28, 2003 the Northampton Board of Health voted to issued a Local Upgrade Approval and also approved your request for a waiver of the garbage grinder regulation and allow the repair of your septic system at 539 Westhampton Rd., Florence, without providing disposal capacity for a garbage grinder. The Board of Health authorized the repair of your septic based on a septic system design, prepared by Robert Sheehan, which will accommodate a maximum flow of 338 gallons of sewage per day. The issuance of the sewage disposal system repair permit is contingent upon the filing of a deed restriction at the Registry of Deeds office, which prohibits the future installation of a garbage grinder in your dwelling, Please submit a copy of the filed deed restriction to the Board of Health office. A Certificate of Compliance for your new septic system will then be issued. Note: For your convenience a copy of a model deed restriction is attached. It is my determination that the Title 5 inspection dated April 22, 2009 may have failed to take into consideration factors other than the septic system itself, including but not limited to the water table and proximate gutters/drainage system. None of the seven existing leaching galleries were checked or excavated during the inspection. With this in my mind, I would like to pursue a second opinion on the matter.
