Mailloux 78 Turkey Hill Road Northampton, MA 01060 Re: West Farms Road, Northampton, MA TCE Project No. 50828 Dear Mr and Mrs Mailloux: On May 2, 1991 John J. Furman under my supervision, performed percolation testing and examined deep observation holes at the above referenced location, in conformance with the Massachusetts Department of Environmental Protection Code 310 CMR 15.00 (effective date 9/9/88) and the City of Northampton by-laws. The testing and examination procedures were witnessed by Peter McErlain, representing the City of Northampton Board of Health. The results of the percolation tests and deep observation holes, in my opinion, meet or exceed the minimum requirements set forth in state and local rules, regulations and standards. A copy of the field noted are attached. Respectfully submitted, Thermo Consulting Engineers wrence R. Smith, P.E. Parsons Mr. Eugene Sheehan Westhampton Road Northampton, MA 01060 Dear Mr. Sheehan: CITY OF NORTHAMPTON MASSACHUSETTS OFFICE OF THE BOARD OF HEALTH September 5, 1984 Re: Sewage Disposal Lot 9 Map 42 210 MAIN STREET 01060 1913)586-6950 Est.213 On September 5, 1984, I observed the excavation of two deep soil test holes (#3 & #4 on the attached sheet) on your property at the intersection of Westhampton Road and West Farms Road (Northampton Assessors Map 42 Lot 9). The tests indicated that the soil consists of medium to coarse sand and gravel, with some cobbles to a depth of 9'6", no ground water was encounter- ed. Based on these observations and the previous percolation tests (#1 and #2 taken April 11 , 1984), it has been determined that Lot 9, Map 42 will support a residential subsurface sewage disposal system when the leaching facility is installed at any of the test sites indicated on the attached sheet. If you have any questions concerning this matter, please contact the Board of Health Office. Very truly truly yoours, //// Peter J.
