Meczywor Board Of Health 210 Main St. Northampton, Ma. 01060 Dear Mr. Meczywor: Enclosed is a copy of the letter sent to all of the tenants at 95 West St.,Northampton. I thought it only fair to give them 30 days to clean out all of the debris. We intend to go into the basement, once it is free of trash,and completely clean the walls and floor with a bleach and water solution and have our plumber install perhaps one or two sump pumps to alleviate the water problem. We will speak to the plumber to see if he feels anything else can be done to eliminate the problem. My husband and the handyman went into Apt. #1 on Saturday and fixed the leak that was coming into the bedroom and replaced the ceiling tile with a new one. I have contacted the state and an environmental agency that will be doing a work up on the mold that may be causing my health conditions. May I please have a copy of your findings and what will be taking place to alleviate this problem. I am now in the process of seeking to relocate our residence as soon as possible. Sincerely, r ; L Joan Fredrick January 28, 2006 Dear Tenants: On Tuesday, January 24, 2006, I met with an inspector from the Board of Health concerning the problem with the water in the basement. It was determined that we have to install sump pumps in the basement to alleviate the problem and also have to clean out the basement completely to avoid any more wet boxes,clothing, etc.which is also causing mold to grow. We have asked for co-operation in the past with cleaning out the basement and now we have to insist that everything is removed by Feb. 28. Anything you want to keep will have to be brought into your apartment or stored in a separate storage unit. Anything left in the basement after Feb. 28 will be brought to the dump. There will be no storage available in the basement, except for bicycles. This will be the only letter I will be sending out concerning this matter. I would appreciate everyone's co-operation. He told me that on April 9, 2007 he changed the filters on the makeup air fan the range hood. He also checked the velocity and flow under the hood and noted that a range hood was working as designed. He also told me that whenever there has been a nolaint in the kitchen he has always responding in a timely manner. never, during his inspection of the makeup air fan to the Dining room, he found out that • fan cage was spinning on the shaft. He started the repair work on April 9, 2007 and npleted it on April 11, 2007 . :lso told him about the back flow problem that PopcoService Inc. noted on the deep ter. He changed the direction on the vent to correct the problem. April 12, 2007, Robert E. Bak, Compliance Officer for Occupational Safety and Health ninistration (OSHA) arrived on site at about 8: 30 am. OSHA received a complaint that are may be an over exposure of Carbon Monoxide (CO) in Lamont Kitchen. . Bak and I went to the kitchen at about 9:30 am. The kitchen was in partial operation the time. We both tested for CO and I also tested for natural gas (LEL) . We both had detection of CO and I had no detection of natural gas. ter talking with the kitchen staff, they told us that the kitchen is usually in full =ration at about 1:00 pm. We also found out that one of the cooks did elect to go to eley Dickinson Occupational Health (CDOH) after the small gas leak on March 28, 2007 and s shift started at 11:00 am. On Wednesday March 28th I attended a class in the morning and returned to Lamont where in the afternoon at about 3 pm there was a heavy odor of natural gas. Since the initial call I put in another work order on Tuesday April 4th since we still smell gas and have not had any follow up to our original call. The supervisor Thursday,April 5th, inquired if anyone has showed up to check the equipment and the answer was NO. Thursday,April 5th,the plumber asked if anyone came to check on the hoods and the answer was NO. The employees continue to smell gas on occasion and continue to feel ill. Lamont's gas appliances are: large conveyor Hobart dish machine, gas hot water booster,gas tilt skittle or grill, 2 gas high pressure steamers,4 gas convection ovens, 2 gas deep fat fryers, 60" gas griddle, one gas over beneath the griddle,4 burner gas stove with gas oven. There are 3 motors running the exhaust hoods and air re-circulation. Last year in the fall the HVAC expert told us the kitchen is under ventilated with an electrical problem in the kitchen where the hoods can't run at higher speed because they draw too much power and cut the electric to the kitchen. There is an under ventilation issue that has been inquired about on numerous occasions and there has not been a remedy. Originally, On March 28th, 10 days ago, I called in a gas leak emergency to Physical Plant job control. I called my department head, Dining Service Director, Kathy Zieja, who advised us to vacate the kitchen and go outside,we the employees of Lamont kitchen opened all windows and doors in the area and vented the kitchen and left before Physical plant arrived,a good 15 minutes later. We left all appliances as they were. 1,1( LIZ.34p t The plummer on call 3:30 pm Brian Pichette, along with Richard(Rich) Korzeniowski, Smith College's Health and Safety Coordinator_ Rich had a monitor(toxicity?) and Brian had a true gas sniffer with the wand. They couldn't find any gas,because the kitchen had been ventilated They then used soap to find bubbles and checked all the fittings and checked pilots. Brian found 2 valves that were loose the - on off gas valves. They were located in the oven under the griddle and in the over of the 4 burner range with self standing oven. Brian tightened them. It took about 1 and''A hours for the gas to build up and them to find the leaks. At this time everyone in the kitchen had head aches, light headedness and scratchy throats;Nancy meuse,Dining Room, Sal Presto cook, I couldn't sleep comfortably that night and had a hard time catching my breath about 2 hours of sleep at a tine. I called in ill the next day at 9:15 am. At I 1:15 am the college called and sent me to hospital because they thought it would be workman's comp related. The hospital tested me for carbon monoxide poisoning and told me it was within range. They did a blood test. The college asked me to come to the office and fill out an accident report and they wouldn't use my sick time it would go under workman's comp. Then Kathy Zieja told me the highest presence of carbon monoxide was found where I stand and work located by the range. They were worried about me. Wanted to make sure 1 would be OK to go to work on Sunday. I filled out the accident report with Ann Finely one of my supervisors. I remain concerned about the working conditions at Smith College and whether there is still carbon monoxide from the ventilation problem in particular and the gas leak hasn't been addressed or fixed to our knowledge. Can you help us since we still feel sick and we don't have answers.
