Dear Mr. Patterson:
On December 21, 2007, representatives from this office,the Fire Department, and the
Building Department conducted an inspection at the premises located at 23 Market Street,
Apartment A, owned, operated or occupied by you,for compliance with CHAPTER II OF
THE STATE SANITARY CODE,MINIMUM STANDARDS OF FITNESS FOR HUMAN
HABITATION, AND/OR MGL,CHAPTER 111, SECTIONS 122-127,ABATEMENT OF
NUISANCES. This letter will certify that the inspections revealed violations listed below,
which are serious enough as to endanger or materially impair the health, safety, and well-
being of the occupants.
VIOLATIONS OF ARTICLE II, 410.000, MINIMUM STANDARDS OF FITNESS
FOR HUMAN HABITATION, AND/OR MGL, CHAPTER 111, SECTION 122-127,
ABATEMENT OF NUISANCES:
410.352, .410.451, & 410.602-The conditions of your apartment were unsanitary with excessive
clutter in every room. The clutter was obstructing all passageways and all of means of egresses.
There is evidence of very poor housekeeping noted upon the inspection. Some food spillage noted
around the apartment. The floors were found to be unclean. The entire apartment is in immediate
need of a thorough cleaning satisfactorily to this Board of Health.
ORDER: Clean entire apartment including kitchen counters, appliances, and floors now and
regularly. Remove and properly dispose of items creating this clutter condition that are of no value.
Remove and disposed of all items creating this clutter condition by disposal or put in storage.
Clean and deodorize all floors and rugs in an approved manner. As a result of cleaning make free all
means of egresses.

Chapter 111, Sections 122-127 of the Massachusetts General Laws,hereby orders you to make
a good faith effort to correct the following violations by December 24 2007 at 12:00o.m.
IMPORTANTANT NOTE:
PLEASE NOTIFY THIS OFFICE AT 587-1214 WHEN ALL OF THE VIOLATIONS
ARE CORRECTED TO ARRANGE FOR A RE-INSPECTION. IF ALL OF THE
VIOLATIONS ARE NOT FOUND TO BE CORRECTED AT THE TIME OF THE RE-
INSPECTION AND ANOTHER RE-INSPECTION IS RE S IRED THERE WILL BE A
75.00 RE-INSPECTION FEE FOR EACH RE-INSPECTION REOIRED.
Failure to comply with this order will result in you being ordered to vacate the apartment or
cause the apartment to be vacated in accordance with M.G.L. c 111 127A and 127B. 105
CMR 400.000: State Sanitar Code Cha iter I: General Administrative Procedures and 105
CMR 10.000: State Sanita Code Cha,ter II: Minimum Standards of Fitness for Human
Habitation. Any and all occupants of the aforementioned dwelling, or portion thereof,must vacate
the premises by December 24 2007 at 5:00 p.m.
Furthermore, anyone who fails to comply with any order of the Board may be subject to fines
ranging from$10-$500 pursuant to 410.910. Each day's failure to comply with an order shall
constitute a separate violation.

On December 21, 2007,I received a telephone message from Russell Jobson of the Hampshire Property
Management Group, Inc., suggesting that I grant you an extension to correct the clutter situation in your
apartment and that three days was to short of a period to do so. Also, on December 24, 2007, during our
conversation about the condition of your apartment, you also requested more time to correct the situation.
Taking both request into consideration,we both agreed to an extension to January 4, 2008,to comply with
the orders of this Department, at which date a re-inspection will be conducted.
Please call this office at 587-1213 on January 4,2008, to arrange for a re-inspection time. Do not hesitate

On January 7, 2008, representatives from this office conducted a re-inspection at the premises
located at 23 Market Street,Apartment A,owned, operated or occupied by you, for
compliance with CHAPTER II OF THE STATE SANITARY CODE,MINIMUM
STANDARDS OF FITNESS FOR HUMAN HABITATION,AND/OR MGL, CHAPTER 111,
SECTIONS 122-127, ABATEMENT OF NUISANCES. This letter will certify that the
inspections revealed violations listed below.
VIOLATIONS OF ARTICLE II, 410.000, MINIMUM STANDARDS OF FITNESS

some improvement shown. There is still excessive clutter in front bed room the middle room and
the kitchen. The clutter in these rooms is still partially obstructing the passageways in the rooms.
There is still evidence of poor housekeeping on your part and noted upon this inspection,which
must improve.
ORDER: Clean the three rooms motioned above to a state that is free from clutter and
accumulation of paper items that are more than 30 days old. Wash all floors in the entire apartment

creating this clutter condition that are of no value. Remove and disposed of all items creating this
clutter condition by disposal or put in storage.
