CONDITIONS: See Attached As agent to the Planning Board I, Carolyn Misch, attest that this is the decision made by the Planning Board at its public hearing May 12, 2016. _________________________________________ May 18, 2016 May 12, 2016 Village Hill Circle Section Approved Waivers  Zoning Waiver to allow minor grading within the Farms Forest and Rivers zone for the rear of lots 11, 12, 13.  290-29 Private Residential Shared Street waiver from the 20’ paved width for the portion of the road that is one-way loop. The one-way portion will be 14’ wide.  290-29 Private Residential Shared Street waiver of 500’ length. Street length is 900’ from Ford Crossing around the loop and back.  290-29 Private Residential Shared Street waiver of two-way traffic direction. The first 215’ will be two-way before the split to the one-way loop portion.   Prior to endorsement: Final plans shall be amended to incorporate all plan changes as required in the conditions and comments from Department of Public Works. Plans shall be stamped by a registered land surveyor and registered professional engineer. Final construction quantities shall be submitted for review and approval by the Board. All additional items must be in the final construction cost calculation. Financial performance guarantee shall be established in accordance with the subdivision regulations. Prior to construction: The applicant must record all covenants related to maintenance of infrastructure and 10’ wide bike/ped sidewalk including snow removal from sidewalks and the path. Private road way and utilities underneath shall be spelled out in owner covenants. Covenants must be reviewed for consistency with the approval and regulations by staff prior to recording. Covenants shall state that this is a private street and future owners may not petition the street for public street acceptance. Easements shall be recorded for the public to pass and repass on the street, the sidewalks, including the 10’ wide bike/ped connection and in the two trail connectors between units 14 and 15 and units 12 and 13. Preconstruction meetings shall be held prior to construction and shall include instructions for control of construction traffic. Prior to the first lot sale or unit sale to any individual homeowner: The open space, shown to be allocated to the City and offered by the applicant must be granted to the City. A sign shall be posted between units #14 and #15 and between #12 and #13 that indicate that there is a public access trail. Prior to a certificate of occupancy for #14, #15, #12 or #13 a permanent public access trail sign shall be installed between these lots. The tee in the water main for the hydrant that will be eliminated shall be replaced with straight pipe. and indicated on utility construction plans. Proposed removal and replacement of asphalt at the intersection of Ford Crossing and Village Hill Road shall be removed and replaced at project completion. An interim trench patch for utility work shall be provided until the permanent asphalt is installed The driveway for lot 21 on Ford Crossing shall be shifted to align with Musante Drive An accessible curb ramp shall be indicated on the northeast corner of Ford Crossing. All proposed crosswalk markings shall be Street Print Duratherm consistent with those throughout the Village Hill development. 5. All water, sewer and drain utilities shall remain private as part of the Private Residential Shared Street. A water gate valve shall be provided behind the sidewalk for the service line to lots 19, 20 and 21. Curb stops for each of these lots shall be provided allowing for each service to be isolated without affecting the others. The new invert for the proposed sewer connection to the existing manhole in Ford Crossing shall match the existing. Revise alignment of the CET lines to eliminate any possible conflict with the signs and stone pillars. The proposed sign with stone pillars shall be a minimum of 5 feet from the proposed fire hydrant and associated gate valve and shall not be so placed as to obstruct access to the hydrant. Fire Hydrants shall be placed at a maximum distance of 500’ apart. All water service curb boxes shall be placed in the grass immediately behind the sidewalk near the lot line. Water and sewer line crossings for mains and services shall be a minimum of 18 inches. Slopes greater than 3: 1 shall have long term stabilization such as armoring. Typical Pavement Section shall be revised to indicate 4 inches of pavement (2.5" base and 1.5" top) over a geotextile fabric with 12 inches gravel in accordance with the subdivision regulations. All details and specifications should be completed in compliance with subdivision regulations and Department of Public Works standards as noted by the Department of Public Works memorandum.
