you are hereby notified to take action to remedy the conditions named below within
TWENTY-FOUR HOURS of the service of this notice, according to Massachusetts General
Laws, Chapter 111, Sections 122-125:
Cockroach infestation in the basement and street level areas at 84 Pleasant Street
(formerly China Chef Restaurant). Continue professional extermination in all areas of the
former restaurant. This extensive treatment will be required to prevent further spreading
of the cockroach infestation into other commercial and residential sections of the building.
In addition, all food products left exposed (Sauces, spices, etc) within the premises must be
removed and properly disposed of.
If at the expiration of time allowed these conditions have not been remedied, or are not in the
process of being remedied, such further action will be taken as the law requires and a fine of
$20.00 per day may be charged.

Dear Sir,
We are writing to inform you that none of the violations cited in your "Order to Correct
dated November 22, 1994 have been corrected by the landlord, Eric Suher,nor by the property
managers, Northriver Real Estate Management. In addition to these outstanding violations, our
apartment has further deteriorated and we believe additional violations now exist- As examples,
holes are now forming in the bathroom floor,broken lock at back door, absence of a working
light in the bathroom and kitchen, and ants in the bathroom. Due to these conditions, we request
a reinspection of our apartment as soon as possible.

Housing Code Violations
A reinspection of the premises at 86 Pleasant Street, #3R was conducted on April 19,
1995 at approximately 9:15 a.m. This reinspection was requested by the occupants of this
apartment and was made as a followup to violations which were noted and ordered corrected in
the Northampton Board of Health Abatement Order(Certified Mail#Z 343 788 668)which
was sent on November 22, 1994. The following was noted at this time:
Violations which have been corrected:
(410.280) Bathroom exhaust fan installed. However, the"pull chain"to the light is missing,
effectively rendering the light and fan inoperable.
(410.480, Back Bedroom- (4) Hole in floor repaired.
410.500 & Kitchen - (2) Hole in floor repaired.
410.501) Kitchen - (4) Pantry door repaired.
Hallway- Deteriorated plaster repaired.
Violations which have not been corrected:
(410.351) Back bedroom baseboard heater cover panel missing. Heater pipe on opposite
side of room with section of pipe exposed along the floor.
(410.480, Back Bedroom - (1) Center prime window not weathertight, ill-fitted, &
410.500& locking mechanism busted.
410.501) Back Bedroom - (2) Left prime window not weathertight, ill-fitted, &
locking mechanism busted.
Back Bedroom- (3) Large hole in ceiling tile.
Kitchen- (1) Both prime windows not weathertight, screwed shut,
sash cords busted.

locking mechanisms are faulty or busted.
It has been reported to the Board of Health that replacement windows for the defective
units noted above have been ordered. However, to date, the Board of Health has not been
notified as to approximate delivery or installation date.

The Northampton Board of Health has inspected the premises at
88 Pleasant Street, Northampton, MA (assessor's map 320 parcel 45 .),
for compliance with Chapter II of the State Sanitary Code.
This letter will certify that the inspections revealed violations listed below,
which are serious enough as to endanger or materially impair the
health, safety, and well-being of the occupants.
Under authority of Chapter III, Section 127 of the Massachusetts General
Laws, and Chapter II of the State Sanitary Code,you are hereby ordered to

absorbent.
