Dear Mr. Adams:
During a perc test on your property at 956 Florence Rd., it was confirmed that your
septic system has failed and must be repaired/upgraded.
The septic system repair/upgrade process is as follows:
• Hire an engineer to prepare plans for the new septic system utilizing the data
resulting from the perc test.
• Submit the septic system plans and a permit application to the Board of Health in
order to obtain a septic repair permit
• When the septic repair permit is issued hire a licensed septic system installer to
construct the new system in accordance with the approved plans.
• Have the newly installed septic system inspected by the design engineer and the
Board of Health.
Please contact me at the Board of Health office with any questions.

On this date the writer inspected the installation of a Soil
Absorption System (septic system) . The writer found the
installation to be complete (except for completion of cover
material and final fill) and in compliance with our plans and 310
CMR 15. 000 . The installer representative (Rich Jaskey) and my
inspection noted that the system was built properly, in
accordance with the state regulations and our plans.
