Please refer to the State Environmental Code Title-5 at 310 CMR 15.302 (3), On January 10,2013, I conducted a joint inspection with Northampton's Board of Health Agents,Mr. Daniel Wasiuk and Mr. Edward Smith. Please note that this house has been vacant since 2011,approximately 1 year before Mr. Stover's inspection, and it has seen very little or occasional use since 2003. This information was obtained from the current homeowner, Mr. Jeremy Ober and the previous Title-5 Inspection report dated December 3,2012. As part of this inspection,three 10' test pits were dug to determine groundwater elevations and to ascertain whether or not the existing soil absorption system is in groundwater. It was nearly impossible to determine a seasonal high water table.The soil next to, and on top of the septic tank, distribution box and pipes leading to the distribution box and SAS was not naturally occurring soil and contained various forms of construction debris and mixed fill (silty soil). There were miscellaneous pieces of wood, bricks,concrete, large pieces of metal and glass,among other things. This material has been on-site for many years. Because this system has not been used since 2011,there was no sign of hydraulic failure above or below ground. No damp soil or ponding. Vegetation was lawn grass. Even though there were redoximorphic colors in the fill, this may be relic mottling or from the solid waste deposited here. The bottom of the existing SAS(3/4" stone) is approximately 46", and below that,the soil is a loose fine sand. Actual groundwater at 23 Fair Street was observed at 6.5'as witnessed by myself and the two Board of Health Agents. The soil logs for 17 Fair Street(westerly side) shows the groundwater to be at 6.5'. The soil logs for 27 Fair Street(easterly side) shows the groundwater to be at 4'. Because of the conflicting groundwater data, it was agreed that a Conditional Pass would be issued at this time. Then, after the tank is full and after 6 months of continued use, another inspection will be required to determine if this system is functioning in a manner that protects public health, safety, welfare and the environment and is not in failure. At that time, and to establish the structural integrity,the septic tank and distribution box will be uncovered to verify their proper operation. I believe I have satisfied the requirements of 310 CMR 15.302 (3). I have met or exceeded the previous Inspectors methods to determine the location and condition of all system components and relevant physical features. Recommendations: Vacate the Title-5 Inspection report of December 3, 2012 and allow the Inspection of January 10, 2013 to supersede said report. If this system is shown to be failing after 6 months of continued operation, a new subsurface sewage disposal system should be designed and installed according to the requirements of 310 CMR 15.00 the State Environmental Code Title-5. Require Mr. Ober to put a deed restriction on this property which limits it to a 3 bedroom house without a garbage disposal untillunless a new subsurface sewage disposal system is designed and installed in accordance with The State Environmental Code Title-5. Approve in writing, {as required by 310 CMR 15.302 (3)}, the Title-5 Inspection report dated,January 10, 2013. The reason for this assessment was due to a Title V Inspection being re-evaluated by Tim Maginnis. The matter was under review and required another site visit by the septic system inspector after 6 months of continuous usage by the homeowner. The d-box was inspected for proper performance and possible failure. The d-box was found to be operating properly with no conditions warranting failure. Also attending this inspection were Jeremey Ober, former property owner, Michael John, Installer and Dan Wasiuk of the Northhampton Board of Health. The purpose of this inspection was to determine whether or not the sewage disposal system at this site was operating properly and not in failure. As a result of this inspection it has been determined that the sewage disposal system at this site is not in failure and is operating in a manner that protects public health safety welfare and the environment. This determination is based solely on the inspection of June 2, 2014. There are no express or implied warranties as a result of this inspection.
