NOTE:
On September 24, 1998, I inspected the work completed through that date by the
contractor constructing the repairs for the 96 Dunphy Drive site. Work completed
included all work as shown on the plans prepared by us as revised in our August 14,
1998 drawings, except for final fill over the leach field to finish grades shown on the
plans. We understand that the final filling and grading was completed as shown on
our August 14 1998 revision.

1 Existing septic tank location unknown, location shown on plans is
approximate, use care when travelirg over area.
2. Place end caps on end of distribution pipe laterals
3. Contractor to notify Engineer no less than 24 hours before pipe, tank & "D"
box are buried with stone.
4. Contractor to obtain two "ties" from each pipe end, "D" box & tank
to house and provide to Engineer for "as built plan".
5. Contractor to remove all soil to top of "C" horizon from leaching area, before
placement of fill and stone for bed, contractor to scarify soil. Contractor to
minimize travel with construction equipment over prepared leaching area.
6. After removal of "A" and "B" horizon soli, if unsuitable soil is found, contractor
to excavate unsuitable soil and fill with clean soil meeting requirements of
310 CMR 15.255(3) to elevations indicated on plans
7. If contractor in doubt about construction methods or details, call engineer
