Leeds, MA 01053 Dear Ms. Mazza: CITY OF NORTHAMPTON MASSACHUSETTS 01060 OFFICE OF THE BOARD OF HEALTH 210 MAIN STREET,Rowe 8 NORTHAMPTON,MA 01060-3167 Re: Septic Inspection, 19 Drewsen Dr., Florence, MA The Board of Health is in receipt of a report on a sewage disposal system inspection conducted at 19 Drewsen Drive, Florence by Sanitarian Tom Leue on June 25,2003. That report indicates that your septic system at 19 Drewsen Dr.fails to protect the public health and the environment as defined in Sec.15.303 of CMR 15.000,State Environmental Code,Title 5. Because the dwelling is located on a street where the city's sewer system is now available, you must, in accordance with the requirements of Mass. General Laws Ch. 83 Sec. 11,connect your dwelling to the sewer line. Therefore, in accordance with the provisions of 310 CMR 15.000 of the State Environmental Code,Title 5 and under authority of Massachusetts General Laws,Chapter 21A, Section 13, and Chapter 83, Section 11,you (or the subsequent owners of the property) are hereby ordered to connect the dwelling at 19 Drewsen Dr. to the city sewer system within two years of the date of the original inspection,(by June 25,2005). If further degradation of the sewage disposal system occurs, (e.g. sewage flowing to the surface of the ground), the connection will be required sooner. In order to tie into the city sewer you must obtain a sewer connection permit from the Northampton Dept. Public Works. Any questions concerning sewer connection should be directed to Asst. City Engineer,Ned Huntley at 413-587-1570,Ext. 107. Please be advised that you are entitled to a hearing on this order to upgrade your subsurface sewage disposal system, provided that you file a written petition requesting such a hearing in the Board of Health office within seven (7) days of the receipt of this notice. Please feel free to contact the Board of Health office, at 587-1213, if you have any questions concerning this matter. Thank you for your anticipated cooperation in this matter. Very truly yours, Peter J.McErlain,Agent Northampton Board of Health cc: Asst. N Any portion of cesspool or privy is within 100 feet of a surface water supply or tributary to a surface water supply. N Any portion of cesspool privy is within a Zone 1 of a public well. N Any portion of cesspool or privy is within 50 feet of a private water supply well. N Any portion of a cesspool or privy is less than 100 feet but greater than 50 feet from a private water supply with no acceptable water quality analysis. [This system passes if the well water analysis, performed at a DEP certified laboratory, for conform bactena and volatile organic compounds indicates that the well is free from pollution from that facility and the presence of ammonia nitrogen and nitrate nitrogen is equal to or less than 5 ppm, provided that no other failure criteria are triggered. A copy of the analysis must be attached to this form.] Y The system fails. I have determined that one or more of the above failure criteria exist as defined in 310 CM 15.303, therefore the system fails. The system owner should contact the Board of Health should be contacted to determine what will be necessary to correct the failure. Title 5 Inspection Form 6/15/2000 page 3 of 9 Homestead Inc.
