The Building Permit was issued by the City. 1-5. The installation of the steel sheeting should be started sometime next week to allow for the concrete pour over the Columbus Day weekend. 1-6. He will witness the placement of the key in the tool box and will lock the tool box with the construction padlock. e.Radiation Therapy will notify Hospital Security that no control is necessary over the vault area and workers may resume the construction.Hospital Security will unlock the access gate to the dig area and take down the Radiation Area sign. HE NOTED THAT THE GENERAL CONTRACTOR DID NOT WANT TO BE INVOLVED WITH ANY LIFTING , MOVING AND PLACEMENT OF VARIAN EQUIPMENTS. THIS IS BECAUSE OF THE COSTLY INSURANCE POLICY THAT WILL BE REQUIRED IN HANDLING THE EXPENSIVE MEDICAL EQUIPMENTS. KI ALSO MENTIONED THAT, THE RIGGING ISSUE FOR THE TREATMENT ROOM DOOR AND FRAME, SHOULD ALSO BE INCLUDED IN THE OVERALL DISCUSSION. THE DOOR MANUFACTURER USUALL HANDLES THE LOADING AND TRANSPORTING FROM THE FACTORY TO THE SITE. A MORE POWERFUL VIBRO—HAMMER WILL HAVE TO BE USED TO FINISH THE SITE SHORING WORK. Patricia responded that a temporary partition inside the Outpatient Waiting area should not be a problem. She needed to coordinate with Scott and Don with the removal of furniture, etc. 3-5. Richard stated that he needed more specific information concerning the Treatment Room door and frame. He needs to coordinate the concrete form and pour with the requirements of the door manufacturer . Richard emphasized that the Hospital would need to purchase the door and frame as soon as possible. He would feel better if they had the frame since it would have to be cast into the concrete . For devices installed as in-plant protection, the reduced pressure backflow preventer shall be installed on the owner's side of the water meter on the potable water supply line. 8. Before installing a reduced pressure backflow preventer, all pipelines shall be thoroughly flushed to remove foreign material. 9. The owner of the device shall be able to shut down water lines after reasonable notice during normal business hours to permit necessary testing and maintenance of the device. If it is not possible to meet this requirement, a by-pass line equipped with an approved type of reduced pressure backflow preventer shall be installed. 10. The reduced pressure backflow preventer and shut-off valves must be installed in horizontal alignment between three and four feet from the floor and a minimum of twelve inches from any wall. 11. If the device is to be installed on a hot water line, a device approved for use at the elevated temperature must be used. 12. If a drain is to be provided for the relief valve port, there must be an approved air gap separation between the port and drain line. To be approved, the air gap must be twice the internal diameter of the discharge line. 13. All water lines shall be color coded according to the state plumbing code, except that water filtration plants, pumping stations, sewage treatment plants and sewage pumping stations shall label all water lines in lieu of color coding.
