regarding the ANSI ratings, maintenance issues and control parameters.
12/21/95 - The automatic door reps presented their information regarding the vestibule
doors to the Hospital and the Architect. The Hospital has accepted the revised Vestibule
layout and the revised floor plan layout. Item closed.

Contractor that no utilities are interrupted without notification and coordination with the
Hospital.

5.1 01/11/96 - The Hospital will notify the Fire Department about the work at the Front
Entrance and the measures being taken regarding emergency egress. The Hospital will
disconnect the fire alarm city box and provide a dedicated telephone line to the street.
5.2 01/11/96 - The demolition will begin on Monday of the Vestibule. By Thursday or Friday
the interior demolition will be near completion. The Contractor is to protect all finishes to
remain.
5.3 01/11/96 - The Hospital will modify the push plates for the access to the ramp to provide
emergency exit egress.
5.4 01/11/96 - The Hospital is implementing ILSM Onterium life safety measures) and will be
conducting extra fire drills, providing additional signage for egress, provide egress maps,
conduct daily checks, and conduct fire drill inservices.
5.5 01/11/96 - The Contractor is expecting to begin sawcutting next Thursday. Demolition of
the Vesitubule may begin at the end of this week. Construction of the vestibule should be
complete by February 5th, with the doors being installed on the 15th of February.

Elaine Kabot will order the extra phone line and the pay phone.

phone, the new house phone, and the reception desk phone will be installed the week of
February 12th. Elaine will be invited to the next job meeting.
6.1 01/18/96 - The General Contractor will prepare the schedule for the replacement of the
storefront window system in order to schedule the painting for the lobby.
01/25196 - The window replacement will begin on the 6th of February, and the Vestibule
framing will be completed and ready for the door installation by the 12th of February.
02/01/96 - The Vestibule framing will be installed tomorrow and the window replacement
will begin the 6th of February. The General Contractor will begin installing the ceiling grid
after the window installation, and prior to the painting.
7.1 01/25/96 - The General Contractor expects to pour the Vestibule slab tomorrow. Framing
for the gypsum board installaton will begin next week and most likely be completed by the
9nth of February.

9.5 02/08/96 - The Architect will contact the cabinet contractor and coordinate delivery dates.
9.6 02/08/96 - The Hospital stresses the need for appropriate safety gear for all who enter the
work area.
9.7 02/08/96 - The electrical rough inspection is expected for Monday.
9.8 02/08/96 - The Contractor provide a requisition, it was reviewed by the Hospital and
Architect and found acceptable.

for the Vestibule will be installed on the 13th. The bifold doors will be installed on the
13th, and 14th. The transom glazing for the doors will be installed by the 26th of February.

the concrete slab will begin tomorrow.

03/14/96 - The heating change over should be at the end of next week.
14.1 03/14/96 - The Hospital has requested that an access panel on the right exterior automatic
door be reviewed by the manufacturer. It was dragging on the slab and was adjusted up by
the maintenance department. Proper closure should be confirmed.
14.2 03/14/96 - The aluminum storefront contractor has and will continue to install the new
storefront windows as the weather and schedule allow. The window at the bottom of the
ramp will be ordered now and the knee wall will be constructed to meet it.
14.3 03/14/96 - The Hospital has requested the electrical diagrams for the automatic bifold
doors. Cut sheets on the doors for general information has been provided by the
Contractor.
14.4 03/14/96 - The Volunteers will not move down into the new lobby space until the project is

14.5 03/14/96 - The Architect will provide an SK drawing detailing the new frost wall at the end
of the ramp to the Contractor, the Hospital, and to the Building Inspection Department.

13.1 03/07/96 - A separation wall has been erected as a temporary partition to separate the
work area from the new vestibule. Due to regulations from the interim life safety measures
adopted by the Hospital, the wall has been constructed to be a one hour fire rated
assembly.
03/14/96 - The General Contractor has prepared a price and the Architect will review it this
next week and prepare a change order for the next meeting.
13.2 03/07/96 - In phase two, the ceiling construction is being removed and the sawcutting of
the concrete slab will begin tomorrow.
03/14/96 - The sawcutting of the slab will be completed by hand next to the brick wall.
The interior brick veneer sits on the slab. The slab should be repoured by the end of next
week.
13.3 03/07/96 - The Hospital is reviewing the existing conditions where they expect to install an
electric unit ventilator.
03/14/96 - Item closed.
13.4 03/07/96 - The lighting and ceiling track and panels are half here for phase two. The other
half should be here shortly.
03/14/96 - The Hospital will confirm arrival times of outstanding items. Item closed.

03/28/96 - The adjustment of the switch has been completed to the Hospital's satisfaction.

windows. Then the drop ceiling can be installed, hopefully, on or before April 1st. The

and 6th.

16.3 03/28/96 - The Architect will contact Amherst Woodworking about field measuring the
"slate" extensions, next Tuesday morning.

04/04/96 - The storefront window installation is complete. Some minor leaking has
occured under three of the windows. The caulking will be reviewed and resealed if
necessary.

16.3 03/28/96 - The Architect will contact Amherst Woodworking about field measuring the

17.1 04/04/96 - The Hospital signed, and the Architect distributed Change Order No. 1. Item

17.2 04/04/96 - The Hospital will call the Mecho Shade company and ask to adjust one of the
adjustable shades, and to schedule the installation of the rest of the shades.
17.3 04/04/96 - The Hospital expressed concern over the impression the old storefront framing
on the existing column, and whether or not the paint will mask the effect. At this time, the
painter will paint the column and see if additional prep is necessary.
17.4 04/04/96 - The painter will touch up the walls, etc on the 15th of April.
17.5 04/04/96 - The Architect will provide a detail sketch to the Hospital outlining a proposed
trim detail for the arch recess. Item closed.

1.3 04/11/96 - Inside automatic bifold doors require alignment and adjustment for smooth and
consistent operation.
1.4 04/11196 - Fix ceiling tile in Vestibule above exterior bifold doors.
1.5 04/11/96 - Replace typical ceiling tiles with coffered tiles as shown on contract documents
on the right side of the lobby. Total of 8 tiles required.
1.6 04/11/96 - Seal saw cuts in existing exterior concrete slab next to Vestibule which were
created when overcutting the slab.
1.7 04/11/96 - Ensure seal on the exterior of the new storefront window systems.

18.5 04/11/96 - The Architect will look at a placement for a clock and directory sign for the
Hospital. Item closed.
18.6 04/11/96 - The Architect recommends that the Hospital seal the exterior grout joints along
the slate sills along the storefront to deter moisture movement into the existing wall. Item

18.7 04/11/96 - This is the last scheduled job meeting. Any additional job meetings will be
scheduled on an as needed basis. Item closed.

04/11/96 - The storefront has been resealed and steps have been taken to eliminate the
leaking around the storefront system. Item transferred to the Punch List.
15.1 03/21196 - The General Contractor expects to have the mason on site next week. Once the
knee wall is in place, the window contractor will be able to complete the installation of the
windows. Then the drop ceiling can be installed, hopefully, on or before April 1st. The
carpet installation is planned for the weekend of April 5th, 6th. Painting should be
complete prior to the installation of the carpet. There will be touch up painting done once
the temporary wall is gone.
03/28/96 - The mason is done, except for an acid wash. The drop ceiling will follow the
installation of the window, today or tomorrow. The area will be ready for painting by the
end of the day on next Tuesday. The painter will be finished painting by the end of the day
on Thursday. The Hospital will contact Bill Morini about installing the carpet on April 5th,
and 6th.
04/04/96 - The mason is done. The grid of the drop ceiling is in. The painting contractor
will be painting today and tomorrow. Carpet installation is planned for Monday, Tuesday,
and Wednesday.
04/11/96 - The painting is completed. Carpet installation will be completed today and
tomorrow. The Electrical work will be completed today. The finish work will be completed
by tomorrow. Item under Hospital control.
16.1 03/28/96 - The temporary wall will most likely come down the morning of the April 5th.
04/04/96 - The wall is slated to come down on Tuesday morning. The ceiling tiles will be
installed that same day. The Hospital will call to coordinate the date with the Contractor.
04/11/96 - The wall is down, a plastic sheet has been installed to prevent odors from
drifting. The plastic will come down when the carpet installation is complete. Item under
Hospital control.
16.2 03/28/96 - Sue Stratton is reviewing the status of the furniture.
04/04/96 - The furniture is here, stored on site. Item closed.
04/11/96 - The furniture for the Upper Lobby will be installed today. The rest of the
furniture will be installed on Tuesday. The task chairs are back ordered, and temporary
chairs will be arriving on Monday. Three benches are back ordered. The Hospital will stay
on top of this subject. Item under Hospital control.
16.3 03/28/96 - The Architect will contact Amherst Woodworking about field measuring the
"slate" extensions, next Tuesday morning.
04/04/96 - Amherst Woodworking has been contacted, and has field measured.
04/11/96 - The oak "slate" pieces have been constructed and are being installed. Item
under Hospital control.
17.2 04/04/96 - The Hospital will call the Mecho Shade company and ask to adjust one of the
adjustable shades, and to schedule the installation of the rest of the shades.

14.1 03/14/96 - The Hospital has requested that an access panel on the right exterior automatic
door be reviewed by the manufacturer. It was dragging on the slab and was adjusted up by
the maintenance department. Proper closure should be confirmed.
03/21/96 - The issue has yet to be fixed. The Architect will contact the door manufacturer
for completion of the installation.
03/28/96 - Item continued.
04/04/96 - The Hospital is requesting the contractor service the doors and to bring the
doors into manufacturer's tolerances.
04/11/96 - The Contractor will contact the automatic door manufacturer and arrange a
meeting with a service representative and the Hospital maintenance staff. Item transferred
to the Punch List.
14.2 03/14/96 - The aluminum storefront contractor has and will continue to install the new
storefront windows as the weather and schedule allow. The window at the bottom of the
ramp will be ordered now and the knee wall will be constructed to meet it.
03/21/96 - Item continued.
03/28/96 - The knee wall is constructed and the window will be installed today or
tomorrow. Item closed.
04/04/96 - The storefront window installation is complete. Some minor leaking has
occurred under three of the windows. The caulking will be reviewed and resealed if
necessary.
