The detection limit for the method is <1% by visual estimation and 0.25% by 400 point counts or 0.1%
by 1,000 point counts.
Due to the limitations of the EPA 600 Method, nonfriable organically bound materials (NOBs) such as
vinyl floor tiles can be difficult to analyze via polarizing light microscopy (PLM). EPA recommends that
all NOBs analyzed by PLM, and found not to contain asbestos, be further analyzed by Transmission
Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is not
covered under NVLAP accreditation.

Regulatory Requirements& Recommendations
A. In accordance with the Massachusetts Department of Public Health, State Sanitary Code,Chapter
II regulations(105 CMR 410.353), every building owner must maintain(and correct) all asbestos
material that may allow the release of asbestos dust, or friable materials, in good repair, and free
from any defects, including but not limited to holes, cracks, tears or any looseness. Any work to
correct the damage must be performed in accordance with all other Massachusetts regulations
applicable to asbestos. Therefore, based on the damaged conditions of the identified ACM's, the
building owner must correct the damage in accordance with all other Federal and State
regulations applicable to asbestos.
B. In accordance with EPA NESHAPS regulations (40 CFR Part 61), building owners must remove
most types of ACM's from a facility(or area of a facility)before any activity begins that would
break up, dislodge, or similarly disturb the material. Asbestos removal is considered as a
"response action" which must be conducted by an accredited (licensed) asbestos abatement
contractor. Additionally, in accordance with the OSHA regulations (29 CFR Part 1926.1101 and
1910.1001), all potential contractors bidding on work must first be informed of the results of this
inspection.
C. In accordance with the Massachusetts Department of Environmental Protection (MADEP)
regulations (301 CMR 7.00, 9.00), each owner or operator must comply with certain procedures
to prevent visible or particulate emissions to the ambient air space. Such procedures include, but
are not limited to, the removal of any asbestos-containing material (both friable and non-friable)
from a facility or facility component prior to demolition/renovation operations if such operations
will cause asbestos emissions, or will render the asbestos-containing material friable, or will
prevent access to the asbestos-containing material for subsequent containment and removal.

As requested by the client, the scope of work did NOT include inspection or testing of other potential
hazardous building materials, such as Lead Paint, Miscellaneous and Universal Hazardous Building
Materials and White Goods, Household/Light Commercial Universal and Hazardous Wastes, above-
ground or underground fuel oil storage tanks, PCB's in Caulk and Joint Sealants, paints and coatings
potentially containing RCRA 8 metals, Mold/Fungi, Radioactive Sources (such as exit signs, smoke
detectors, and Radon), Urea-formaldehyde Foam Insulation (UFFI), or Pigeon & Bat Waste, or oil and
hazardous material releases or threats of releases (310 CMR 30.000 or 40.0000). The inspection was
limited to building materials recently subject to disturbance by recent renovation activities, and only
those materials reported to the Asbestos Inspector by the Owner/Client that are subject to renovation
disturbance [sheetrock,joint compound walls (excluding the ceiling), plaster walls, 9" floor tiles/mastic
in the rear room, exterior renovation debris pile, and exterior siding materials.
Labeling and signage for identified or presumed asbestos-containing materials was not included in the
scope of work. Areas behind or inside permanent walls, columns, ceilings and floors were not inspected.
All areas that were unsafe to enter, including areas that were structurally unsound or in danger of
imminent collapse, were be inspected. The following common inaccessible areas were not inspected or
sampled as part of the scope of work: Wall and floor utility chases (unknown to exist); Pipe Tunnels
(unknown to exist); Crawlspaces, Air plenums above permanent ceilings, including above or behind
plaster or concrete, etc.; Below-ground or below-grade (buried areas; parking lot, driveway, slab or
foundation damproofing, etc.); fire doors. Additional areas and materials not inspected or sampled
include the following:
• 1F Porch (all wood construction). Suspect exterior window glazing on two (2) wood window
sashes not subject to planned renovations per Owner;
• Newly installed sheetrock wallboard, flooring sheets, and joint compound throughout the 1St and
2nd floors. Owner to obtain Safety Data Sheets (SDSs) from the renovation contractor. Materials
purportedly purchased at Home Depot;
• Suspect linoleum flooring/adhesive and 12"floor tiles/mastic in the 1st floor kitchen;
• New aluminum replacement windows throughout;
• Potential adhesives on wood paneling in 1'floor living room;
• 1St floor Rear Bathroom(gutted with all new construction materials);
• 1st floor Main Bathroom(gutted with all new construction materials);
• 2°d floor throughout(gutted with all new construction materials);
• Non-suspect wafer board material in exterior debris pile.
Note that the PLM/DS Method can fail to detect asbestos fibers present in vinyl floor tiles because the
fibers used in vinyl tiles were typically "small", present in relatively low concentrations, and hard to
extract from the vinyl matrix. For this reason, Forbes & Wheeler recommends that when the standard
PLM/DS method fails to detect asbestos in vinyl tiles, the sample be further analyzed by destroying the
interfering matrix materials and isolating any asbestos fibers that may be present. The components
remaining are then recommended to be analyzed by Transmission Electron Microscopy(TEM).

Dear Mr. Goff:
At your request, on November 13, 2013, Forbes & Wheeler, Inc. conducted a limited asbestos inspection,
including bulk material sampling and analysis, of portions the above-referenced facility and debris. The
purpose of the inspection was to sample building materials recently subject to disturbance by recent
renovation activities, and only those materials reported to the Asbestos Inspector by the Owner/Client that
are subject to renovation disturbance [sheetrock, joint compound walls (excluding the ceiling), plaster
walls, 9" floor tiles/mastic in the rear room, exterior renovation debris pile, and exterior siding materials.
The work was performed as a result of potential enforcement activity from the Massachusetts DEP.
Regulated asbestos-containing materials (ACM's) that were identified are to be properly cleaned,
removed, and disposed of, as required by applicable Federal and State regulations.
As required by Federal and State regulations, sampling was performed by an EPA-accredited and State-
certified asbestos inspector. Sampling was done in a manner to prevent airborne fiber release and were
placed in appropriately labeled containers that were sealed and submitted to an independent laboratory for
petrography analysis using the EPA-endorsed Polarized Light Microscopy with Dispersion Staining
(PLM/DS)method, using an improved method for analysis (EPA Method 600/R-93/116: "Method for the
Determination of Asbestos in Bulk Building Materials"). The percentage of asbestos present in each
sample was determined by the visual area estimation technique.
Summary of Findings
Please refer to the attachments, including the Asbestos-Containing Materials Inventory and laboratory
report and chain of custody for a complete listing, description, locations, and quantities of each identified
individual type of ACM and Non-ACM encountered, sampled, and analyzed.
Limitations&Exceptions of Inspection
Inaccessible areas behind or beneath the surface of the debris pile were not inspected. No heavy equipment
was used to uncover any inaccessible materials that may be suspect of asbestos. Therefore, it is possible
that not all building materials or debris were sampled.
